On behalf of the National Quality Center, we are excited to continue onward with the end+disparities Learning Exchange; one of our most innovative and important improvement initiatives to-date in partnership with the HRSA HIV/AIDS Bureau. Partners in+care is the consumer involvement aspect of the Learning Exchange. Over the course of the exchange, we will focus in on how to involve the consumer community in this important work based on our monthly overall topics.

The end+disparities Learning Exchange a 9-month initiative that promotes the application of improvement interventions to reduce HIV-related disparities in four key subpopulations while building and sustaining a community of learners among Ryan White HIV/AIDS Program recipients. The Learning Exchange offers informational opportunities by content experts and utilizes the power of peer learning to facilitate local improvement efforts on eliminating HIV-related disparities. Communities of Learning: collaborative, QI campaign, Regional Groups
Consultation: On/off-site coaching of recipients to advance their clinical quality management programs Together, we can make a difference in the lives of people with HIV. NQC provides assistance to RWHAP recipients to improve HIV care since gaps in HIV care still exist and advances are uneven across HIV populations. 250 recipients participated in 28 Regional Groups 51% of all recipients joined the largest HIV QI campaign; viral suppression increased from 70% to 76%, a statistically significant improvement Training and Educational Fora: monthly webinars, advanced trainings, online QI tutorials 40% of RWHAP recipients received TA and 95% would recommend TA to others 40 online QI tutorials are available; over 35,000 have been taken so far Intensity Information Dissemination: monthly newsletters, websites, publications, exhibits, QI awards Michael NQC is funded by the HRSA HIV/AIDS Bureau since 2004 and provides technical assistance to all RWHAP recipients to build their sustainable clinical quality management programs. Over 90% of all recipients have accessed intensive NQC offerings and take advantage of our various services. The Learning Exchange is an extension of these offerings. After that, we will open up the webinar for your questions in our Q&A session.

Learning Objectives Describe the process to become a PLWH Stigma Index project site. Name one or two early findings from the stigma index assessment in NJ. What is Sustainability?
Deloris and Michael tag team: D: Ask the audience if they know what we mean – have someone (or maybe two if the first response is short) explain what they think. M: Tell the audience that sustainability is making sure that the work we do makes a lasting impact. For example, it’s easy to see progress in something we are focused on and spending resources on. But what happens when we decide its time to move on and focus on something else? Is there a way we can prevent ourselves from losing the gains we see around our work? How can we do this without having a never ending story for each project we complete? Continued on next slide…

Details on Sustainability
Draft a Sustainability Plan Continue to measure performance, but less frequently Periodically check in with Quality Improvement team to assess their impressions of sustainability vs backslides Share results around the clinic and demonstrate how things are continually getting better Find common connections to other challenges and try to grow from one project directly into the next using the connections in subject matter D: First, come up with a plan and write it down. This is the best way to make sure none of the detail is lost. This is usually the last step in any improvement process – a closeout and sustainability plan. M: Second, performance measurement is your best friend. Once you move on to another topic, make sure that the performance is continually measured. Perhaps it doesn’t need to be measured weekly or monthly, but rather annually or biannually just to check in to make sure there isn’t a backslide. Make sure this is detailed in the sustainability plan. D: Third, keep talking to the folks who had been the core members of the QI team. This doesn’t have to be too frequent – perhaps monthly or quarterly huddles to check in on the team and to see what they feel has happened since transitioning from active to sustaining phase. M: Fourth, keep the subject matter alive (or extend its life), by spreading good results around the clinic or network to make sure that new opportunities to spread learning D: Does anyone else have anything to say or add about sustainability?

Ann Bagchi, PhD Dwight Peavy Rutgers School of Nursing
New Brunswick, NJ Dwight Peavy Community Advocate Newark, NJ Deloris introduce Ann: Dr. Bagchi has more than 15 years of experience conducting HIV-related health care service and policy research and has worked with people living with HIV in a clinical capacity for three years. Her recently completed Doctor of Nursing Practice quality improvement project examined barriers to, and interventions to improve, routine HIV screening among primary care providers. She has been involved in several project related to addressing HIV-related stigma, including a pilot study to implement stigma-reduction workshops in a local HIV clinic, an online survey on HIV-related stigma among healthcare workers throughout the state of New Jersey, and the New Jersey Department of Health-funded implementation of the People Living with HIV Stigma Index. Dr. Bagchi is also involved in policy work at the local and state levels, serving in leadership positions of the Newark Eligible Metropolitan Area/HIV Health Services Planning Council and the New Jersey HIV/AIDS Planning Group’s Stigma Workgroup. Deloris introduce Dwight: Dwight has worked in the HIV/AIDS arena for more than 20 years. He served as Executive Director, for the Newark EMA HIV Health Services Planning Council for 12 years. Dwight is currently working as a consultant, on various HIV related projects. He is one of twelve international advisory members with the International Network for Strategic Initiatives in Global HIV Trials, also known as INSIGHT, representing New Jersey and Washington D.C. He is a member of the Advisory Board associated with INSIGHT and serves as one of the two national science representatives that contribute in setting the science trail agenda. Dwight also serves on the Board of Trustee to the FORUM for Collaborative HIV Research in Washington D.C. Dwight is one of the founders and Chair of the AAOGC Board of Trustee in Newark , New Jersey, and chair of the New Jersey HIV/AIDS Planning Group Prevention and Care Collaborative Committee. He chaired a diversity task force group that submitted a report and recommendations to a local Newark, NJ AIDS agency to address cultural sensitivity in delivering services to clients in the organization. Peavy serves on several Boards throughout the state of New Jersey. He is the coordinator of Morristown Community Health Day celebrating their 22nd year of providing free medical screenings to low income Morris County residents. He has given several lectures and facilitated workshops, seminars and conference, and is well known in the community for his advocacy for people living with HIV/AIDS. Ann and Dwight – (15-20 min) share the process you took to become part of the PLWH Stigma Index how has this process gone in NJ? Anything to expect or what out for? what are some of the early results? what are you doing with these results (are they shared)? what are you doing to reduce or address stigma for PLWH in NJ as a result? if you had to do this all over, would you? Would you do anything different? what are the first steps you recommend for anyone wanting to do this work?

Deloris, introduce Aaron who will talk about expanding projects that work: Aaron Burgess is a first-year doctoral student and research assistant at Florida International University in Miami, FL, where is he studying Social Welfare. He earned his MSW (Master of Social Work) degree from University of Maryland, Baltimore in 2014 and holds an active graduate-level social work license in Washington, DC. Aaron has a passion for research related to sexual/gender minorities and HIV-related health disparities and his current doctoral research is focused on suicide prevention with sexual and gender minorities. Aaron has been awarded the following awards for his advocacy for sexual/gender minorities and persons living with HIV: the Distinguished Mountaineer Awards by WV Governors E.R. Tomblin in 2011 and J. Manchin in 2012 as well as the DC Concerned Providers Coalition Advocacy Award in 2015. In his private life, Aaron is a former Zumba instructor and aspiring novelist. Aaron - Here are some talking points (15 minuteS) Tell us about the program you ran at Washington Childrens Hospital What was it? What was involved? Whom did it serve? How did it come to be? What were the patient impressions (experience)? Do you have any info on how health outcomes were affected for participants? How did the program grow/expand over time? What advice would you give someone wanting to do something similar in their location? Are there any concrete first steps you recommend? Anyting to expect? Anyting to avoid?
