1 JABSOM Annual Faculty Report Instructions

2 Notes All reporting is for academic year (July 1 – June 30)
Reporting is for all compensated faculty only Faculty with joint appointments should complete a report for your primary department only You will need a UH username/password to log in All text comments need to be kept at a 500 word limit

3 Need a UH account? UH username/password is required to log-in to the reporting system If you have forgotten your username/password or need to obtain one go to the following website: UH ITS Set-Up Please note that your UH username/account will be issued directly by UH ITS Dept –Contact the UH ITS Help Desk at for assistance

4 To Begin Your Reporting Session
Go to the Faculty Development Website: Click on the “Annual Faculty Reports” tab Click on CLICK HERE TO LOG IN TO FACULTY REPORT SITE 

5 Log-In Screen You will then see the log-in screen.
Log-in with your UH username and password (same as your UH account)

6 CV You may upload a copy of your current CV
This will be helpful so that you may copy and paste information from your CV into your Annual Faculty Report form This will be used to help you and your department chair keep track of your academic contributions and progress towards promotion/tenure You will have to upload a new version of your CV every time you update it

7 Annual Faculty Report To help the school produce collective faculty data required for reporting purposes for school, university, legislative and accrediting body reports May be used by your department chair as a means to provide information as related to meeting the needs of the department and to monitor individual progression towards applying for promotion and/or tenure

8 Annual Faculty Report Please be aware of the data being requested, in the block columns to the right, please indicate a numerical number and not text

9 Annual Faculty Report Be as accurate as possible
When unsure, please make your best estimate Each year’s report will be stored for future reference Goals from last year will automatically be moved to this year’s goals Publication section will be blank and needs to be updated each year Information in grants section will carry over for the remaining period of the grant award

10 Reminders Click on the “save” button at the end of each section to save your work At the end of your session, you must click on the “submit” button to submit your form You may submit/change your data as many times until the deadline Upon clicking “submit” a printable version will appear Click on “Click to Print” button to print a copy for your files Click on “log out” to end your session If you need to make changes after submitting your report, please contact Lori Emery at or to re-open your report
