Slip Trip Fall Injury A member of Thames Water supply chain had been requested to undertake CCTV survey of sewer section on Elmstead Lane, BR1. The operative stepped out of his vehicle onto grass verge and into a concealed hole (covered by grass,, causing a twist to his ankle. Causation factors The crew had identified slips, trips and falls on their on-site risk assessment, but had failed to fully check the environment in which they were working in, especially with regard to access and egress to their vehicle. The location was not well lit, and the evening was becoming dark. What can I do differently? Operatives must ensure that they accurately assess the working environment (access/egress), not just the task they are doing when completing on site risk assessments and setting up working area. Operative are to ensure working area has appropriate lighting. Tony Ashford Learning from Incidents
