Patient-Centered Care How can the government ‘mandate’? Outline

PAPER STATE EMR STATE Patient information is documented and stored on a database. Any authorized doctor or nurse has access to all patient information and can make informed decisions about the patient’s care. A typical visit to the doctor is documented on paper then stored on shelves. Anything about the patient is copied and put into this file. Any decisions made mean the files has to be found and reviewed or asked of the patient.

Paper State Issues Poor Handwriting
One chart copy; unable to easily share vital information Patient is required to repeat their information to every nurse and doctor. Laboratory, radiology and other test results have to be mailed, faxed or sent on paper to be added to file. The hospital doesn’t have the MD records; the MD doesn’t have the hospital records. Paper State Issues

Lab tests are repeated unnecessarily
Doctor #1 Bloodwork Doctors do not know the medications or tests ordered by the other doctor. Radiology The emergency room treats the patient without access to any of the doctors’ office information. Doctor #2 Lab tests are repeated unnecessarily Paper State Issues

EMR – BENEFITS Records are clear and legible.
All care providers have access to medical history, allergies, and current medications. Reduces errors of duplicate prescriptions or medications that are not compatible. Reduces redundancy of tests. Shares important data, like immunization records. Will allow the patient a soft copy of their own medical record. Prescriptions are sent electronically to the pharmacy of the patient’s choice.
