29 SMART AIM: By February 2nd, 2016 we will increase our breastfeeding initiation rate of women with intention to breastfeed from 49% to 70%. To test multiple proposed changes to increase our breastfeeding initiation rate of women with intention to breastfeed from 49% to 70% by February 2nd, 2016 We will measure: N of pregnant women who report intention to breastfeed at enrollment and at 36 weeks gestation or more; N of women who have infant feeding plan with written goals; N of women who received professional support; and N of women who initiated breastfeeding at delivery. Nurse home visitors will provide breastfeeding education to all pregnant women at enrollment and at 36 weeks gestation or more, identify those with intention to breastfeed and encourage the completion of an infant feeding plan with written goals before delivery, identify women who need peer counselor or professional breastfeeding (IBCLC) support, and identify women who initiate breastfeeding at delivery. They answered the 3 questions from the model for improvement

30 Breastfeeding: PDSA Cycles
Cycle 4: How many women with intention to breastfeed initiated breastfeeding? 11 out of 14 women 78% Cycle 3: Verify how many women with intention to breastfeed at 36 weeks or more were contacted by IBCLC: 1 out of 7 women 14% Cycle 2: Verify how many women who reported intention to breastfeed at 36 weeks or more have an infant feeding plan and written goals. Study revealed 100% Cycle 1: Verify how many women at enrollment and at 36 weeks or more who have the intention to breastfeed. 100% reported intention to breastfeed.

31 What We Have Learned The nurse home visitors provided breastfeeding education to 26 pregnant women; 24 women reported intention breastfeed while 2 women had no desire to breastfeed; 6 women are 36 weeks gestation or more. 100% of the women at 36 weeks or more reported intention to breastfeed. 8 out of 8 (100%) women with intention to breastfeed at 36 weeks gestation or more have an infant feeding plan with written goals before delivery. Only 1 out of 7 (14%) women at 36 weeks gestation or more was identified with the need for professional breastfeeding support (IBCLC) and she received a phone contact from IBCLC. However, a NHV referred a new mom who has initiated breastfeeding to IBCLC and home visit was conducted by IBCLC. 11 out of 14 (78%) women with intention to breastfeed at enrollment and 36 weeks or more, initiated breastfeeding at delivery; 2 did not initiate breastfeeding (1 contraindication and 1 changed her mind); 1 was discharged from the program before delivery. This data represent total deliveries in the 1st 90 days. Providing breastfeeding education at enrollment provides pregnant women the opportunity to learn about breastmilk and its advantages. Breastfeeding education at 36 weeks or more reinstate interest and promote intention to breastfeed. Infant feeding plan and written goals assist pregnant women to plan ahead like keeping a journal. Nurse home visitors with breastfeeding education impact their clients with adequate knowledge that influence the intention to breastfeed. The provision of adequate information and guidance in decision making are essential in breastfeeding.

Identify appropriate indicator(s) (in addition to HV-CoIIN measures): 100% of staff will increase their knowledge from less than 70% to more than 80% using the Wellstart Breastfeeding training. Identify if target HFA breastfeeding mothers are experiencing sore nipples, low milk supply, and/or inability to identify infant feeding cues. Develop tool to track efficacy of interventions. There is not a steady trend of Mom’s Initiating BF. The prediction is that When all HVs use the Infant Feeding HV more women will initiate BF. The average amount of weeks that Moms are BF is 10.5 weeks. The goal With the Infant Feeding HV is to have More Mom’s sustaining BF.

In total we had 6 mom’s deliver or due to deliver and we completed 5 feeding plans (83%).

Our goal is to collect data on 100 percent of new enrollees and 75% of participants at 36 weeks of pregnancy. Identify appropriate indicator(s) (in addition to HV-CoIIN measures): We collected qualitative data from the nurses to determine whether the tool was positive. 100% of the nurses said that the tool opened dialogue with their clients and was a positive experience. 100% of new clients will be asked intent at intake and 36 weeks pregnant. At this time we have maintained 100% Changes being tested to meet 90 day goals & collaborative aims: Creating a questionnaire to answer intent and duration of breastfeeding in all of our clients Training all of the staff on how to use the tool when working with their clients. Will the tool impact the amount of time a nurse spends with a client and will it impact how they interact with the client. Next Steps: Try with more nurses and clients and add a pre/post assessment for both nurses and clients. ADD IN YOUR RAMP OF TESTING IF YOU HAVE MULTIPLE CYCLES OF TESTING – EXAMPLE OF ANOTHER PROGRAMS RAMP BELOW Pick one of the changes from the previous slide. Show how the your team has tested the change over time, differing the conditions (day of the week, who tests it, population, etc.) and scale (amount of time, number of home visits, etc.) By 1/9/15 two HVs will develop a decision tree & protocol for an “Infant Feeding Home Visit” and test it with 1 family each. By 1/31/15 all home visitors will test the “Infant Feeding Home Visit” with clients approaching 36 weeks gestation. Cycle 2: Testing combined feeding and delivery plan by two nurses. Both had 2 deliveries and of the 4 clients, 2 are exclusively breastfeeding. Cycle 1: Testing borrowed feeding plan by two nurses. It was well-received, but clients needed more support at time of delivery.
