Macedon Ranges Health is registered with NDIA to provide the following services: Assist – Personal Activities – Daily living skills development to support independence and Choice Health and Wellbeing Assist Life Stage - Transition and Life skills in home and Community

3 NDIS services provided at MRH
Group/Centre Activities Plan Management MRH Support Coordination Assistance with Self Care Activities Assistance to Access Community, Social and Recreational Activities

4 Finalising NDIS provider status for Early Childhood Services:
Paediatric Speech Therapy Paediatric Psychology Specialist Group early childhood intervention (social skills groups) Paediatric physio from 10 years > Paediatric Occupational Therapy Continence Nursing – SWEP funding applications, assessments, in home support

5 Finalising NDIS provider status for Early Childhood Services:
Nursing – general Group and 1:1 childhood specialist groups Paediatric Podiatry – managing pain, footwear (orthotics, custom footwear and sports) ingrown toenails, growth problems (pigeon toes, general growth) milestone checks), gait retraining ( modifying walking patterns) in clinic or outreach support

6 Access to services Participants wishing to engage Macedon Ranges Health (MRH) as their preferred service provider, will be supported by the NDIS Service Engagement Officer at MRH. Participants to phone MRH on , complete an Expression of Interest with the receptionist over the phone, or your planner can contact MRH on your behalf.

7 Access to services Referrals can be received from GP, family, participants and planners Expressions of Interest (EOI) will be forwarded to Central Intake for the NDIS Engagement Officer. Referral can also be received via fax, Connecting Care and face to face and on MRH Website – e referrals

8 Service Agreement MRH Officer will contact participants within 2 to 3 days for high priority (urgent) referral or within 7 working days for non-urgent enquiries An appointment to be arranged for meeting to discuss service plan and sign MRH Service Agreement – this meeting can take place in the home or at MRH.

9 Service Agreement Service Agreement will be explained to participate including billing each f/n, non-attendance policy, staff Log, complaints process and cancellation of services. Once services are agreed upon – Engagement Officer has sighted/has a copy of NDIS Service Plan, coordination of services will begin.
