HSE LEARNINGS INCIDENT: Root Causes: LEARNING:
Twenty four Galfar employees out of 144 residents in TAC, Nimr reported to Galfar clinic with symptoms of abdominal pain and diarrhoea. The were treated with diarrhoea tablets and ORS powder. The symptoms subsided within nine hours. Root Causes: Biological infection from food resulting in mild poisoning Inadequate frequency of internal inspections and audits of HACCP. Quarterly and annual inspections and audits are not effective Hygiene maintenance of camp facilities is not supervised for quality and effectiveness LEARNING: Establish an assurance mechanism for compliance to HACCP (Codex) and Hygiene standards Key Learning: Check the expiry dates on foodstuff purchase and consumption.
