Our Profile “Big County, Big Heart, Big Opportunity”
About Us: Multi-sectored, community coalition covering large geographic area charged with reducing chronic disease rates and improving access to care for 4 million residents, with emphasis on equity and impact. A young coalition of three years, we are relatively new to the collective impact scene with no dedicated funding source. A shared vision and high levels of trust and transparency fuel our work. Actively evolving efforts to efficiently address disparate populations and working to increase integration of individual partner efforts using social network analysis. Seeking: A community that has experience in working on similar large-scale initiatives, creatively adapting to an environment with limited resources and has experience in creating realistic, time-bound shared metrics.

A growing movement among our cities to create supportive built environments and community design for healthy, active living. A strong commitment to creating policy, systems, and environmental changes that support tobacco-free living in Maricopa County. A grassroots effort to provide a linkages to care safety net to ensure health care is accessible and affordable to all.

Evaluating our partner network utilizing the Social Network Analysis tool to assess effectiveness over time. Providing communication, data, and technical support to our network partners to encourage a community of learning and understanding of best practices. Engaging stakeholders and building a cross-sectored partnership with high levels of trust and transparency. Moving from cooperative and coordinated activities to more integrated initiatives among our partners. Developing shared measures with short-term and intermediate outcomes to better champion our “wins” along the way. Identifying paths to financial sustainability!

Charlie Brown is an iconic character who represents the average person, with good days and bad days. He tends to overthink things a bit, but he hopes for the best and tries as hard as he can to accomplish things. He never gives up!
