Immunisation Process LMC – GP Maternity Record NIR nomination
Overdue babies will appear on ImmsNet at 10 weeks, 4 months and 6 months Practices must complete ImmsNet reports within 2 weeks If a child is referred to Outreach or rescheduled, they will not appear on ImmsNet - practices are encouraged to set internal reminders/alerts so they are not overlooked SNR faxes from NIR must be actioned promptly to facilitate urgent Outreach follow-up Casual babies: follow practice protocols. Send precalls and recalls to encourage on time immunisations Red cells represent system safeguards, the others are best practice LMC – GP Maternity Record Records nominated GP Sent to NIR NIR nomination Sent to GP inbox Accept newborn nomination Register Baby Enrol under B code or confirmed enrolment Set immunisation recalls Send welcome letter and precall at 4 weeks Text reminder or phone call – 5 weeks 6 week immunisation Baby vaccinated Book for 3 month immunisations Precall 11 weeks Or First Recall 7 weeks Second recall 8 weeks Refer to Outreach by 9 weeks ImmsNet reminder at ~ 10 weeks 3 month immunisation Book for 5 month immunisations Precall 19 weeks First recall 14 weeks Second recall 15 weeks Refer to outreach by 16 weeks ImmsNet reminder at 4 months 5 month immunisation First recall 23 weeks Second recall 24 weeks Refer to outreach by 25 weeks ImmsNet reminder at ~6 months Note: This process applies to casual babies if an immunisation has been given at your practice ADHB and WDHB Immunisation Process Flowchart V Final 1.1, March 2015
