1 Example 4: (A,B) Standard CC and MLO views of the right breast in this screening mammogram for a 60-year-old woman who never had any prior mammograms. There is a large, dense, partially visualized oval mass in the outer deep mid right breast (arrows), the visualized margins of the mass appear well demarcated (notice the similarity to the mammographic findings on prior study). The study was reported as a BIRADS 0 and the patient was called back for obtaining additional views and/or ultrasound. (C) An exaggerated CC view failed to show the posterior margins of the mass. (D,E,F) US images of the mass shows a corresponding large heterogeneous irregular mass (arrows) invading the pectoralis muscle (asterisk) with posterior acoustic enhancement and focal areas of increased blood flow. At that point and since findings were highly suggestive of malignancy, the BIRADS classification was upgraded to 5 based on ultrasound features and a biopsy was obtained that confirmed mucinous carcinoma of the breast. The posterior acoustic enhancement, although atypical with malignant neoplasm, is seen in this case as a result of the mucinous component. In this case the diagnosis of mucinous carcinoma was suggested based on the typical imaging characteristics prior to pathological diagnosis.
