1. HealthWise HRA and Wellness Screening Program
Program opens 8/1/2017 and ends 12/1/2017
New Partner – Quest Diagnostics (both health questionnaire and wellness screening)
Program is voluntary, confidential, and available to all Crown employees and spouses enrolled in a Crown Health Plan
Employee participation can earn $130 in 2018!
Must complete the health questionnaire prior to 12/1/17
Must complete a wellness screening prior to 12/1/17
$130 HealthWise Credit is applied in $5 added per pay ($5 x 26 pay periods = $130)
Only spouses enrolled in a health plan can participate, just don’t earn the $130 HWC
Only requirement to earn the $130 in 2018 is to completed health questionnaire and blood draw prior to 12/1/2017
Enrollment in a Crown Health Plan is NOT a requirement this year.
Incentive based on participation only and NOT how an employee scores

Health Questionnaire – online – opens 8/1/2017

Wellness Screening – includes a blood pressure check and blood draw for cholesterol profile (total cholesterol, HDL, LDL, triglycerides) and fasting glucose
Wellness Screening Options
Onsite at Branch –Your Branch Screening Info Here
At a local Quest Patient Service Center
Submit lab results (cholesterol profile and glucose) from a blood test you previously completed in 2017 to HealthWise

Detailed report both online and mailed to home
Online report allows you to “move the dial” on wellness screening numbers to let you see impact to body

4. Exciting Features with Quest
Fax to Physician
You can add up to 2 physician fax numbers during the scheduling process
Quest will then send lab report to physician
MyHealth Profile 16-page booklet will be mailed to home
