You are NOT required to complete the survey. The survey should only be completed by students who are 18 years of age or older.

Guide for Survey Write your name on ONE of the two envelopes.
Open the envelope that does NOT have your name on it. Write the 4-digit number in the envelope on the page 3. Do survey. Return the worksheet with the envelope that has your name on it.

Guide for Quizzes Problems on quizzes are based on the textbook exercises. Every quiz is of 10-point scale. Approximately there are 3 to 4 problems at each quiz. Quizzes are on every Thursday.
