Columnar cell lesions and atypical ductal hyperplasia. Columnar cell change is characterized by columnar change of the luminal cells within the terminal-duct lobular unit (TDLU). There is preservation of luminal polarity and lack of atypia within the nuclei (A). In cases associated with flat epithelial atypia and atypical ductal hyperplasia (B, C), there is focal loss of polarity within the proliferation associated with cellular stratification and monomorphism (flat epithelial atypia), and rigid epithelial arcades and micropapillary tufts (atypical ductal hyperplasia).
