3 Collaborative Aims At the end of this Collaborative, the following will have been achieved: Strengthened partnerships across Parts as evidenced by established communication strategies for the purpose of collaboration for quality management; state-wide quality management priorities; and joint training opportunities to avoid duplication. A portfolio of performance measures will be in place for strategic planning and quality improvement processes and data are routinely collected based on established data collection methodologies. A unified statewide written Cross-Part quality management plan will be in place for each participating State; supported by a work plan for implementation. At least one joint quality improvement project initiated by cross-Part teams. Routine Cross-Part quality management assessments.

4 Participant Expectations
Pre-Work-activities to prepare for the first Learning Session. Aim Statement in line with the goals of the Collaborative and with the current quality management mandates in mind. Learning Sessions-participate and share what is learned Commit Resources- time; leadership; energy Identify expert staff (i.e., financial, information system, and clinical policy development) to assist the team on an as needed basis. Perform tests of change in the state that lead to widespread implementation of improvements across Parts, aligning quality improvement activities across constituencies. Collect and Report Collaborative measures bi-monthly for the duration of the Collaborative. Share information with the Collaborative, including details of changes made and data to support these changes at Learning Sessions and during Action Periods. Fun- Enjoy the learning experience

5 Faculty Expectations Coaching- Offer on-site and off-site coaching to participants Guidance- Provide a quality improvement framework to assist participating teams in their efforts to build a statewide quality infrastructure to align quality improvement activities. Knowledge Management- Provide good ideas to test for improvements in quality of care. Connector- Provide communication strategies to keep participants connected to the Faculty and colleagues during the Collaborative. Subsidy- NQC will cover travel and lodging costs for team members to attend Learning Sessions.

6 Key Collaborative Terminology
Learning Sessions: Face-to-face or virtual meetings with all 5 State Core Travel Teams to Learn from expert Faculty and peers/colleagues Network with Faculty members and colleagues across the country Gather new knowledge on the subject matter and process improvement Share experiences and collaborate on improvement plans Problem-solve improvement barriers Action Periods: The time between Learning Sessions when Collaborative Team Members work to implement improvements in their states Teams Focus on their own state, but remain in contact with other state teams, staff, and Collaborative Faculty Teams share results of their improvement efforts in bi-monthly reports Participation in Action Period activities is not limited to Core Travel Team members.
