2. THE COMMUNITY PARTNERS PROGRAMME (CPP)
As we get older we may choose to get support in the home or in the community.
Later we may want to live in an aged care facility.
Organisations that provide support to the elderly are called “aged care support service providers”.

3. WHAT DOES THE CPP WORKER DO?
The CPP worker is the link between the aged care services and the German community
We tell aged care services about the German community
And we tell the German community about aged care services

4. WHY?
SO THAT ELDERLY GERMAN PEOPLE HAVE GREATER ACCESS TO CULTURALLY APPROPRIATE AGED CARE SUPPORT SERVICES
HOW:
The CPP worker gives information to the German community on aged care services so you can make the right decisions
AND
The CPP worker teaches Aged care services about the German community and their needs so they understand you better

CPP workers to:

- Provide German cultural information kits
- Teach Aged Care Services about
German culture and the needs
of elderly German people
