Facility Security Plan
FSP- Valid for 5 years FSP Review/Approval: Facility Security Assessment, CG6025 and 2 FSPs. FSP Amendment – All changes

FSP FSP must be audited annually
If there is a change of owner or operator If any modification is made to the facility

Facility Response Plan
Valid for 5 years Substantial (Mobile): 5 years from submittal Significant and substantial Facility: 5 years from COTP approval. FRP review/approval: submit 1 FRP

Facility Response Plan
FRP must be reviewed annual. Review to include any revisions including listing of fish and wildlife and sensitive environments identified in the ACP in effect 6 months prior to plan review.

FRP All changes must be submitted for amendment.
All amendments required COTP approval except for revisions to personnel and telephone numbers. If no revisions are required. It must be documented in the annual review on the record of changes page.

Operations Manual Operations Manual review –
Submit LOI and 2 Operation Manual. Valid until the facility seize to operate.

Operations Manual All amendments must be submitted for approval.
Personnel and telephone number lists do not require examination by the COTP.

Applicability A port, terminal or shipyard:
receives oceangoing tankers, or any other oceangoing ship of 400 gross tons or more. (OIL) receives oceangoing ships carrying NLS. (Noxious Liquid Substances) receives oceangoing tankers, or any other oceangoing ship of 400 gross tons or more; or fishing vessels which offload more than 500,000 pounds of commercial fishery products from all ships during a calendar year. (Garbage)

OIL COA Facility must: Receive oily mixtures with in 24 hours.
Complete reception of ballast water containing oily mixtures within 10 hours. Complete reception of other oily mixtures within 4 hours

NLS COA Facility must: Receive NLS Residues within 24 hours.
Complete reception of NLS residue within 10 hours

Facility must: Receive APHIS Garbage no later than 24 hours.
Garbage COA Facility must: Receive APHIS Garbage no later than 24 hours.

Applying for COA Facility must: Fill out the appropriate form/s. A-Oil, B-NLS, C-Garbage Submit reception facility agreement.

COA To continue to receive ships, a port or terminal must hold a COA.
Must be available for inspection. A copy must be in the Operations Manual COA is valid for 5 years
