After You Enroll Enroll in FSA during the Washington College Open Enrollment period Prior to 1/1/2018, you will receive a Participant ID Notice via or mail Tip: Save your notice! You’ll use this to create a log-in to our web portal, flores247.com where you can manage your account. If you enroll in the Medical FSA, you will also receive a Flores benefits Mastercard Cards will be loaded with your entire Medical FSA election and you can begin using it on your benefit eligibility date. Your card will be valid for several years, so please do not destroy your card after your balance is spent. When you re-enroll the next year, the same card will be reloaded. TIP: If you need extra cards, you can order them on flores247.com under the “Maintenance” Tab Tip: Save receipts for services purchased with the debit card in case you receive a debit card documentation request

“No-wait” dependent care Process
If you enroll in Dependent Care FSA, you can take advantage of our “no-wait” dependent care claim process and receive automatic reimbursements for dependent care expenses every pay period! Why is it called No-Wait? If you file a claim for your yearly dependent care expenses at the beginning of the year, each pay period your reimbursement will be issued to you automatically, meaning you don’t have to wait to see your money hit your account! How to file your claim: Log-in to using your Participant ID number Go to the “Document Library” to download and print the Dependent Care Claim Form Complete the form and have your provider sign to verify your expenses or provide an itemized receipt Return your form to Flores electronically or via fax/mail and you will automatically receive your reimbursement every pay period! Due to our quick and easy reimbursement process, dependent care funds are not linked to a Benefits debit card.

What is an Flexible spending account (FSA)?
A Flexible Spending Account (FSA) allows you to set aside pre-tax dollars to pay for eligible medical and dependent childcare expenses. Each pay period your pre-tax contribution is deducted from your paycheck and deposited into your FSA. Depending on your tax bracket, you may save from 25% to 40% by participating in this benefit.

Medical FSA Basics You can contribute up to $2,600 to a Medical FSA for the 2018 plan year to pay for your eligible medical expenses, as well as expenses incurred by your spouse and qualified dependents during the plan year. Services (or payments in the case of orthodontia only) must take place between 1/1/2018 and 12/31/2018*. Eligible Expenses include**: Medical: copay, deductible, and co-insurance Prescriptions Vision: exams, prescription glasses, contacts ,LASIK Orthodontia payments Dental: cleanings, fillings, and crowns. Prescribed OTC medications Medical Supplies *If you enroll in the FSA after 1/1/2018, your medical FSA balance will be available on your benefit effective date. Example: If you are hired 1/15/2018 and benefits are effective on your hire date, your enrollment period for the FSA would be 1/15/2018 – 12/31/ Please check with your HR department to receive your specific benefit effective date if you are a mid-year enrollee. ** If you enroll in the High Deductible Health Plan option with an HSA, the Medical FSA may only be used for dental and vision expenses.

How to Access your Medical FSA Account Balance Reimbursements are issued by check, direct deposit*, or by using your Flores Benefits MasterCard. The entire Medical FSA balance is available for reimbursement on 1/1/2018*. Grace Period and deadline The Medical FSA plan allows for a 75-day grace period. If you have not spent your full balance by the end of the plan year, you can continue to spend down your account through 03/15/2018. All claims must be submitted by March 31, 2019 in order to be reimbursed. *Direct deposit requires a 10 business day pre-note period. If you enroll in the FSA after 1/1/2018, your medical FSA balance will be available on your benefit effective date.

Dependent Care FSA Basics
You may elect to contribute up to $5,000 to a Dependent Care FSA for the 2018 plan year. The funds contributed to this account may be used to pay for eligible childcare expenses you incur for any dependents under the age of 13 so that you, and your spouse if married, may go to work. Eligible Expenses Include: Daycare or Preschool Before and after school care Day camps Ineligible Expenses: Overnight camps Tuition for Kindergarten and above Educational Expenses Fees not applied toward the care of the child How to Access your Dependent Care Balance Complete a Dependent Care claim form and submit this with the childcare provider’s signature or invoices to Flores. Reimbursements are issued on a daily basis via check or direct deposit if you have set this up! All claims must be submitted by March 31, 2019 in order to be reimbursed.

Look for your PID Notice to arrive in the mail or , and create your log-in for to manage your account Look for your benefits card to arrive prior to 1/1/2018 (or before your benefit effective date for mid-year enrollees) and order additional cards on the Maintenance tab at (after log-in) Download our E-Receipt app in the Apple or Android device store If enrolled in dependent care, complete and return your No-Wait Dependent Care Claim to Flores and submit direct deposit information so we can begin automatically reimbursing you in 2018!
