Create a PERSONAL health plan foldable to prevent heart disease. See example The further down the foldable the more important this practice is to you. Be specific in your plans. Start with the one MOST important to you.

6 Section Criteria Smoking: One way to prevent second hand smoke, One Reason Smoking hurts the cardiovascular system. Cholesterol: list at least 2 ways YOU will maintain healthy blood pressure. P.E.: How many hours/ week. List what YOU would do in those hours. Stress: 3 techniques and reasons why they would help YOU relieve stress. Food: 3 of YOUR favorite heart healthy foods/ ingredients and a recipe.
