A systematic method for providing nutrition care for patients, clients and groups Four steps: Nutrition Assessment Nutrition Diagnosis Nutrition Intervention Nutrition Monitoring/Evaluation

4 Nutrition Assessment ABCD’s Anthropometrics
Biochemical data, tests, procedures Clinical information/nutrition-focused physical findings Diet-related history (and other pertinent client history)

5 Nutrition Diagnosis Standardized language used PES statements Use of nutrition diagnosis may vary by site What do you know about nutrition diagnosis terminology? Resources available to you to write PES statements?

6 Nutrition Intervention
“The Plan” Should address nutrition diagnosis Stay curious about the root of the nutrition problem before jumping to intervention stage

7 Activity PHC/Interior Heath In-patient acute care (patient is eating) VCH Outpatient counselling FH/PHSA Seeing a client in their home (community nutrition) Island/Northern Health Intensive care (patient in a coma) What might be some nutrition interventions to address a nutrition diagnosis of: “Inadequate oral intake”? Hint: also think about what the causes (etiology) might be in these situations

8 Nutrition Monitoring and Evaluation
Key questions to ask: Is your intervention working? Is the intervention impacting your nutrition diagnosis? What can you do to measure progress/outcomes? Think back to your ABCDs (clinical settings) Revisit project objectives/outcomes (MGMT, PPH, Research)

9 Documentation What kind of documentation is used at your core site?
Similarities to NCP? Differences? ADIME Follows NCP SOAP Used by many professions S and O together captures the assessment data A can include diagnosis P often includes intervention, monitoring, and evaluation

12 Patient Situation Mr. A 55 year old, admitted with recurrent urinary tract infections, long history of type 2 diabetes Ms. B 33 year old, transferred from ICU, originally admitted after a car accident, in ICU for 1 month Mr. C 70 year old, newly admitted, had a stroke, history of HTN Mrs. D 45 year old, admitted for IV antibiotics, has been in hospital for 2 weeks Ms. E 89 year old, admitted after a fall at home, diagnosed with failure to thrive

17 Designated weekly half-day
Key reminders Designated weekly half-day Every Thursday afternoon of the internship year (excluding the orientation module) is a designated half-day of non-placement time This weekly designated time is intended to be used to: (1) work on Research Module or other module activities, (2) attend education sessions organized by sites (when applicable), or (3) attend education sessions organized by the UBC Dietetics Program (when applicable) If there are no activities or project work planned on a particular Thursday, interns can use this time for self-directed learning

Key reminders Supervision expectations From Role Delineation document (posted online): “Preceptor is responsible for supervising intern during placement” Note: If preceptor is off-site during any part of a placement, a conversation about an alternate supervisor (if necessary) and appropriate activities to engage in should occur
