Our 3 Medical Plan Options---ACO network in Lincoln HSA (Health Savings Account) review Teladoc—Preventive Care---Emergency Room usage Carrier change for the Life and Disability plans including new guarantee issue amounts and reduction in waiting period for STIR. New Parental Leave benefit New VTO policy Voluntary benefits---cancer & accident 2.There are no benefit changes to our 3 medical plan options – we do have an ACO network now in Lincoln Health savings accounts review and funding. Great way to save for medical expenses – tax free! We have other options for doctor’s care with Teladoc The life and disability plans and leave policies will now be with Reliance Standard. This is any important time for the voluntary life offering. Only now at Open enrollment can you elect voluntary life insurance without Evidence of Insurability. After 1-1, you will be medical underwritten. There is also Guarantee Issue for spouses. The disability plan has been enhanced too. Immanuel is offering new parental leave policy and new volunteer time off policy. Doug Keeler will review the voluntary cancer and accident plans. 3 Let’s review the 3 Medical Plan choices:
Deductible $2,500 ($5,000 Family) Coinsurance after Deductible 100% Maximum Out-of-Pocket $3,425 ($6,850 Family) After reaching deductible, the plan pays 100% except you pay the following copays until your maximum out of pocket is reached. Primary Care Visit $25 after deductible Specialist Visit $50 after deductible Emergency Room $150 after deductible Urgent Care $75 after deductible Out-Patient Therapy Rx’s $10/$35/$65 after deductible **Select Choice POS II when doing a Doc search on Aetna.com 3.No plan design changes - deductibles and out of pocket expenses remain the same. Medical Plan 1 offers Aetna’s broad network (Choice Plus). The deductible is $2500 single and $5000 family. Once the deductible is met the plan pays at 100% for most of services. You will see that there are copays for primary care, specialists, ER urgent care and Rx. These copays do accumulate towards your maximum OOP. This plan is a qualified HDHP and can be paired with Health Savings Accounts. We will review HSA’s in a few slides.

Hourly EEs making $15 per hour or less get a 10% discount on their premiums. As a reminder, these premiums are deducted on a pre-tax basis – you save Federal, state and FICA taxes (at least 25%). *Note: Hourly EEs making $15.00 per hour or less get the subsidized rates.

5 ACO – What does it mean? Accountable Care Network
ACO network models offer a member-focused, doctor-driven approach to rewarding health systems who successfully manage the health of employee groups. Electronic health data is on a shared network system within an ACO network. Can help on coordinating care and cutting down on duplicate tests and possibly conflicting RX issues which can add to your costs or inhibit good health care.

Medical Plan 2 is a qualified HDHP and can be paired with a Health Savings Account. The deductible is $1500 single and $3000 family. Once the deductible is met, most expenses will be paid at 100% except for the copays listed below. The provider network is the ACO and is more limited than the broad network in Plan 1. To find providers in the ACO, search Aetna’s website for Whole Health Network.

Medical Plan 3 is a traditional plan with copays before the deductible. Copays start day one. The deductible is $2500 single and $5000 family. Once the deductible is met for services such as hospital, surgeries, MRI’s, the plan pays 70% and you pay 30%. Copays apply towards physician visits, ER, Urgent Care, and Rx. This plan is not a qualified HDHP so you can not pair with HSA. The network for this plan is the ACO limited network. The network on the AETNA website to find providers is Aetna Whole Health.

For Medical Plan 3, the wellness regards are credited to your per pay period premiums. We talked about Medical Plan 1 and 2 and how HAS’s can be paired with these plans. Immanuel will match $ for $ - up to $350 for single and $1077 for EE+dependents. Plans 1 & 2 also provide $377 for the year in wellness rewards if you meet requirements of our wellness program and earn points. This is $14.50 per paycheck. Why HSAs? The money deposited in your account is tax deferred – contributions going in, monies earned and monies used for qualified expenses are never taxed. The monies can build up for when you have medical expenses tax free. Optum is our HSA vendor. The IRS places limits on how much you can save each year in your HSA for 2018 it is $3,450 single and $6,900 for family. Please remember that the wellness credit and Immanuel’s match are included in these limits for savings. The $ for $ match is a great benefit – something that most employers do not match. This benefit really does make the Immanuel plan different and better than the competition.

Save time & money on unnecessary ER/Urgent Care visits with convenient phone or video consults Board-certified doctors diagnose, treat & prescribe medications (if necessary) for common health issues, like respiratory infections, allergies, short-term prescription refills & more It’s easy to get in touch: Visit Call Teladoc ( ) Upload the mobile app available through the App StoreSM or Google PlayTM 10. Teladoc is a virtual visit available on all three plans. You can talk to a doctor any time, any where for common health issues like respiratory infections, allergies, short-term refills, pink eye, etc. Plus this will save you time and money – ER and Urgent visits are very expensive. Teladoc also has a mobile appl. The cost is up to $40 on Plan 3. Plan 1 and 2 are $0.

12 Preventive Health Care Paid for by our Immanuel Medical Plan @ 100%
Preventive Health Care Paid for by our Immanuel Medical 100% **Effective July 1, D Mammograms are now allowed for preventive and diagnostic use under the Aetna Mammography policy. Annual Physicals Mammograms Well-woman care (pap smear) Diabetes screening Well baby & child care Prostate cancer screening Colon cancer screening Adult & childhood immunizations 11. All three plans cover preventive care at 100%. We have one new coverage – 3D mammograms will now be allowed for preventive and diagnostic testing. We wanted to share our utilization of preventive claims for this year through August 31. You can see that the percentages are low of claimants to actual eligible members. Please get you appointment set for your routine exam. All of these tests are covered at 100% - there is no reason not to go – your biometric wellness screening is important but these visits are very important too. When you set appointment, please remind providers office that this is a routine exam so they file claim appropriately to Aetna.

13 Emergency Room/Urgent Care/Teledoc
As a group, we continue to see a large number of emergency room visits. Average cost of an Emergency Room Visit is $1,350. Average cost of an Urgent Care Visit is $120. Average cost of a call to Teledoc $40 or less. We all need to continue to be good health care consumers and keep the costs down for ourselves and our health plan! 12. Immanuel wanted to share the utilization of Emergency Rooms for 2017 through August. Compared to last year’s utilization for the same timeframe, visits are up 53%. Please consider using Urgent care or Teladoc. Emergency visits are very expense and are for life threatening conditions. 51% Increase

14 Ameritas Dental Plan Annual maximum ($1,200) is ALL for dental expenses. Same Plan Design & Same Carrier---Ameritas There is incentive coinsurance which increases from 80% (1st year), 90% (2nd year) & 100% (3rd year). Adult and Child Ortho 50% up to $3,000 lifetime maximum ($1,500). There is a dental reward carryover from year to year with dental rewards up to $250.

15 Ameritas Dental Plan You earn dental rewards by submitting at least one claim for dental expenses incurred during the benefit plan year, while staying under total claims $500 in the same year. The one claim could be your preventive check-up. The annual carryover amount of $250 is added to the following year’s maximum of $1200 the first year. Additional bonus of $100 is earned if you see a PPO provider. The accumulated maximum carryover amount is $1000.

17 Changes to Life/Disability Plans
Moving from Hartford to Reliance Standard. No EOI up to the new guarantee issue of $200,000 for Employee and $100,000 for Spouses with this Open Enrollment period only. Waiting period change for STIR changed from 3 week wait to 2 week wait. New carrier Reliance Standard for life disability and STIR. – This is also a true Open enrollment – Now purchase life insurance without EOI – Guaranteed issued – Only this one time. After 1-1, health questions and you could be declined coverage if any medical conditions. 20 New Parental Leave Benefit
Immanuel will provide up to 2 weeks (80 hrs.) to eligible employees following the birth or adoption of an employee’s child. This benefit will be effective on or after January 1, 2018. Eligible employees are: Worked for Immanuel for at least 12 months Worked at least 1,250 hours in the previous 12 months Benefit eligible (28 hours) employee Parental leave is paid at 100% of the employee’s regular pay. 17. 21 New VTO Policy Immanuel’s Mission is “Christ Centered Service to Seniors, Each Other and the Community”. Immanuel’s desire to model Jesus’s example of service and described serving as “the one who is greatest among you shall be your servant.” Benefit eligible employees earn one day (8 hours) each calendar year beginning in January of each year. Time off may be taken in either a full-day (8 hours) or a half-day (4 hour) increment and is paid at the employee regular rate of pay. Examples of appropriate uses for VTO include: Volunteering at a food bank Cleaning up a park or trail Serving as a mentor or coach through a mentorship program Volunteering at a local hospital Volunteering at a school Examples of inappropriate uses for VTO: Taking a ski vacation and charitably giving ski lessons Attending your child’s PTA conference Judging a beauty pageant Attending a professional, religious, or personal interest conference 18. Identity Theft value add with Reliance Standard The first thing I would do with this new benefit is to go online and protect your credit cards, passwords, etc. in their digital vault – WalletArmor incase you would lose your wallet or purse. The walletArmor monitors the internet’s underground economy 24/7 and will alert you if any sensitive information is compromised. InfoArmor also provides assistance if you are a victim of identity theft. They will help resolve ID Fraud and work with creditors to clear your name.

28 Reminders: Open Enrollment is October 30, 2017 thru November 20, 2017
This is the only time you can make changes to your benefit plans during the year unless you have a family status change such as birth, marriage, spouse’s loss of job to name a few. HSA contribution amounts and 401k contribution amounts can be changed during any payroll period in the year. You can enroll online with UltiPro or thru the Call Center. Please be proactive and make sure you have the cards you need for the benefit plans you select before you are in the doctor’s office.

29 Reminders Cont: If you elect the Flexible Spending accounts for Medical or Dependent Day-Care or Limited Medical, you must reapply for calendar year Medical limit is $2650 and Dependent Day-Care limit is $5000. Remember to turn claims in for Wellness Reimbursement on the cancer policy--$50 on the Low policy and $100 on the High policy. Outpatient physician reimbursements on Accident policy--$25 on the Low policy and $50 on the High policy. Reminder to start earning your Wellness points for the Wellness Cycle (150 pts). Also required are the tobacco affidavit; biometric screening and Health Risk Assessment.
