Aim of the Frailty service @ PAH
To provide a more comprehensive specialty focused service for our older patients To reduce the number of our older population spending a long time in the Emergency Department To facilitate a reduction in length of stay for patients and enable them to return home sooner with the right care

Inclusion Criteria over 75 years Clinical Frailty Score of ≥4.
Patients presenting with falls/reduced mobility/breakdown of package of care/dehydration/UTI/Constipation/SOB/COPD/ chest infection/confusion/dementia Patients requiring pain/pharmacy management Patients who can be discharged same day or requiring up to 72 hours inpatient care

Exclusion Criteria Patients who can be treated within the community via Single Point of Access Patients who clinically need an acute admission ie #NOF, head injury Patients who could be directly admitted to a Community hospital

What is a CGA? Comprehensive geriatric assessment (CGA) is a multidimensional, interdisciplinary diagnostic process to determine the medical, psychological, and functional capabilities of a frail elderly person in order to develop a coordinated and integrated plan for treatment and long-term follow-up.

The Frailty Score PAH Frailty Service is for patients with a >4 frailty score

Frailty Assessment Unit
Service commences Monday 17th October 2016 Located within PAH on ward B40 Open 09:00 – 17:00 Monday – Friday Patients accepted via GP Co-ordinator or from PAH Emergency Department via bleep 755 The team: Frailty Lead – Janice Bernardo (3/10/16) Frailty Consultant – Dr Azad Harripaul Frailty General Manager – Ali Hearn Ward Manager – Peter Robinson Therapies support – Susan Fullen

How do you refer? Contact PAH GP hotline – and ask for the medical GP Co-ordinator. You will need the following information regarding your patient: What is their Frailty score? What are their current obs? What is the referral reason? Is the patient able to make their own way to PAH? Have you considered all alternative community pathways via single point of access?
