3 STAR ASSESSMENTS Saturday December 2, 2017 – 12:30pm
Saturday, December 9, 2017 – 8am Parents will receive a letter regarding specifics for that day. Make Up Dates & Times Tuesday, December 5, 2017, 4pm Saturday, January 13, 2018, 8am Parents will need to register their students for the makeup session.

4 Before the Transition IEP Meeting
Forms/Items to complete before the Transition Meeting: Complete OPRF enrollment forms. Pay school fees, this includes students who are privately placed. Make an appointment for a freshman physical. Bring medical diagnosis forms should your child have a medical diagnosis in the IEP.

5 What to Expect at the Transition Meeting
Students should attend with parents. Arrive minutes early to park and enter building. Meetings are on the OPRF Bell Schedule Times will be as follows: 8:00AM, 8:53AM, 9:51AM, 12:30PM, or 1:23PM. OPRF will be sending parking passes through D90 and D97. Please be sure to place it in the rear window of your car.

6 Who Attends the Transition IEP Meetings
D97 and D90 Staff will share information about students to help guide the D200 IEP Team in identifying the students Least Restrictive Environment. Items discussed will be: Possible Courses based off of recommendations Related Services (Speech/Language, OT, PT, Counseling, Vision, Hearing) Goals Accommodations Transition Plan (started at 14 ½ years old and continues until the student graduates) Special Education placement and related services

7 In the classroom we…. Provide structure/support for adolescents who have learning or emotional challenges which interfere with their academic progress. Provide support for successful integration into the general education academic environment. Focus on improving study/organizational skills. Encourage use of strengths to overcome challenges.
