CP-Net: working together to improve quality of care! During meetings and on a web-based forum, knowledge-brokers, persons with CP/parents and researchers share knowledge, experiences and best-practices, with a focus on ‘doing’ in practice. Results Outcome of the implementation of four recommendations from the updated Dutch Guideline for Diagnosis and Treatment of Children with Spastic Cerebral Palsy (June in 20 collaborating centers, with 30 locations in The Netherlands. STAGE OF IMPLEMENTATION OF FOUR RECOMMENDATIONS IN 29 LOCATIONS OPTIMIZING HAND FUNCTION: FUNCTIONAL THERAPY, TASK-ORIENTED THERAPY, BIMT AND CIMT STOP USE OF BOTULINUM TOXIN TO OPTIMIZE HAND FUNCTION Transitie APRIL APRIL 2017 APRIL APRIL 2017 GUIDANCE OF ADOLESCENTS WITH CP IN THEIR TRANSITION TO ADULTHOOD USE OF THE CLASSIFICATION FOR FUNCTIONAL COMMUNICATION (CFCS) APRIL APRIL 2017 APRIL APRIL 2017 The stage of implementation for each recommendation in each team is registered using a simple color system; green (‘doing’; fully implemented), orange (‘knowing’; team knows what to do and how), and red (‘orientation’). Conclusion The model of collaboration in CP-Net leads to actual changes in clinical practice based on evidence based guidelines. The system supports transparency and collaboration. Follow-up is necessary to support ongoing implementation of guidelines and improvement of quality of care. Relevance and Impact The mission of CP-Net is that every person with CP should have the opportunity to be treated in their own environment according to the best evidence.
