Young people use self harm, via cutting, overdosing & ligaturing as a coping mechanisms. The team wanted to provide better support, outcomes and education around the management of these behaviours.

Background
The UK has the highest self-harm rate of any country in Europe with estimates of 400 in 100,000 people self-harm. These figures are likely to be higher as many people who self-harm do not tell anyone about it. Self-harm can affect anyone however the majority of people who report self-harm are aged between 11 and 25 – Mental Health Foundation

Every year in the UK there are 150,000 cases of Sepsis, resulting in a staggering 44,000 deaths (UK Sepsis Trust, 2016)

This strategy includes the following groups for whom a tailored approach to their mental health is necessary if their suicide risk is to be reduced:
children and young people, including those who are vulnerable such as looked after children, care leavers and children and young people in the youth justice system
people with untreated depression
lesbian, gay, bisexual and transgender people
One of the most effective ways to prevent suicide is to reduce access to high-lethality means of suicide. Suicide methods most amenable to intervention include:
• hanging and strangulation in psychiatric inpatient and criminal justice settings;
• self-poisoning

Reduce levels of self harm by providing coping mechanisms and tools as alternatives to self harm via cutting & overdosing.
Provide appropriate & reliable education tools to staff & young people within the inpatient environment.
Improve outcomes in young people, focusing on self harming and overdosing behaviours by March 2017.
Provide sepsis prevention, recognition and treatment awareness for young people and staff.
Trust wide review of ligature management led by frontline clinical staff including in house training & roleplay review of incidents.
Implementation of a continued care approach to overdosing behaviours to improve patient outcomes & patient experience.
Explore and develop a process of preventing A&E admissions typically around Paracetamol O/D and developing a protocol to manage these in house when safe to do so (with a clinical evidence based approach).
Proactive use of observation and. engagement
Reduce levels of self harm at high risk times by reviewing the roles of staff during this time and engagement from young people.
Provide activities during high risk times for young people.

Implementation of clinical improvement board & sepsis awareness theme for staff education.
Review of the twilight role and activities after 5pm.
Exploring face to face formal training in ligature management for inpatient staff & developing an E-Learning package for all staff to access.

9. Update
Continued implementation of coping skills books and coping skills boxes: with funding being sought for this project.
Attempts to arrange and plan a training session for all staff in wound management.
To develop a in-house process around overdose & physical health management to present to the trust for review.
