4 HIT achievements Expert care, closer to home RENOIR
RENOIR have achieved all they set out to do, so have ‘closed’ as a HIT – their work is now embedded in the Bristol Eye Hospital’s day to day work

5 HIT achievements Dementia diagnosis moved to primary care Dementia HIT
Dementia diagnosis rates have increased from 49.8% in Bristol and 47.8% in South Gloucestershire in , to 70.4% and 60.7% respectively in January this year

6 HIT achievements Fundraising, design and development of Bristol Brain Centre MOVE HIT Bristol Brain Centre opened in 2016, bringing together Bristol’s considerable strengths in neurology together under one roof. Also houses Dementia HIT.

7 Looking ahead Refreshing the partnership agreement
Aligning Bristol Health Partners strategy and priorities more closely with HITs Taking account of system priorities –new Sustainability and Transformation plan Strengthening sponsorship role of Host HIT organisations Current partnership agreement ends in 2017, so this is an opportunity to align Bristol Health Partners strategy and prioritiesmore closely with the HITs

8 An Apple a Day exhibition at Engine Shed
2016 Bristol Health and Care Awards 3,000 attend Bristol’s first Healthy City Week 250 attend the health day at Festival of the Future City
