The EQ-5D-5L tool is a Patient Reported Outcome Measure (PROM).
Completing the questionnaire helps collect information about the service user’s perspective on their wellbeing. Answers to the questionnaire can help identify goals that are relevant to individuals’ recovery.
Adding PROM information to RIO enables us to record each client’s view alongside the rest of their notes. Comments can be copied into care plan documentation or CPA documentation as appropriate. The next pages give step by step information about completing RIO EQ-5D-5L PROM forms.
Paper copies are still used to collect information with service users.

1) On the right hand side of the service user’s case record page, select Docs: Outcomes Measures from the Mental Health Services dropdown menu.
2) Choose EQ-5D-5L from the submenu.

3) Select referral & date/time of PROM offer if known, or todays date. The referral field will also auto-populate. Check it is right for your team
4) Complete the thermometer or VAS (visual analogue score). This score autofills from the previous record so check it is correct for the current rating. You can indicate if the service user declined this question.
5). A PROM review date can be entered if there is a provisional review or discharge date (e.g. CPA meeting or end of therapy) when the PROM will be offered again

6) Complete the form with the information reported by service user……Individual questions can be declined or completed..
Additional comments, clarifying service user’s concerns can be recorded and copied into care plan letters or progress notes using hyperlinks at the bottom of the page.
7) Save the form.
8) Record “PROM offered” in Progress Notes.
The Index field autocompletes when all 5 domains are answered.
