1. Guidance for completing HAO PROM on RIO
The Hope, Agency and Opportunity questionnaire (HAO) is a Patient Reported Outcome Measure (PROM).
Completing the questionnaire helps collect information about how successful we are at delivering Recovery orientated services. The questionnaire can also help identify goals that are relevant to individuals’ recovery.
Adding PROM information to RIO enables us to record each client’s view. Comments can be copied into care plan documentation or CPA documentation as appropriate. The next pages give step by step information about completing RIO HAO PROM forms.

2. Guidance for completing HAO PROM on RIO
1) On the right hand side of the service user’s case record page, select Docs: Outcomes Measures from the Mental Health Services dropdown menu.
2) Choose HAO form from the PROM submenu.

3) Select referral & date/time of PROM offer if known, or todays date.
These fields autofill from previous PROM record, check they are right for your team.
4) A PROM review date can be entered if there is a provisional review or discharge date (e.g. CPA meeting or end of therapy) when the PROM will be offered again.
5) Complete the point in the care pathway at which the PROM was offered.
6) You can indicate if the service user declined to complete PROM. This helps keep track of who has had PROM offered, even if there’s no more information collected.

4. 7) Complete the form with the information reported by service user……Individual questions can be declined or completed. Additional comments can be recorded and copied into care plan letters or progress notes using hyperlinks.
8) Save the form when you have completed adding information.
9) Record “PROM offered” in Progress Notes.
