Purpose Increase States’ knowledge around collaboration and cross-boundary work as it relates to Transition. Panel will offer specific examples of collaboration between Centers and solicit information from the audience State Teams will engage in discussion with and learn from each other about current transition efforts including promising practices and challenges.

Reaching Consensus Reach consensus in your own system first
Know what your system is interested in attaining Have a clear idea of what you can bring to the table as leverage Have the right players at the table to establish agreements

Use initial meeting to establish a shared goal and a specific goal based strategy Clearly define the role of each collaborative member Discuss resources, services and funds each partner can contribute toward accomplishing the identified goals Establish a timeline linked to specific outcome goals

Sustaining Collaboration
Define a level of systems impact substantial enough to warrant ongoing contribution Establish a method to measure systems impact that is relevant to all contributing parties Develop a sustainability plan in conjunction with the work plan and roll them out at the same time
