It is that time of year that we review the state requirements for students with life-threatening health conditions. We will also discuss personal safety which includes blood borne pathogens and updates on the H1N1 (Swine) Flu. Successfully prepare all students for their future

4 Purpose of Orientation
Familiarize each employee with the district’s safety policy and All aspects of the safety program as required by Washington State Department of Labor and Industries Every employee must receive an orientation. Successfully prepare all students for their future

KSD Self-Insurance The Kent School District is a self-insured employer for L&I claims in the state of Washington. This means that if you are injured on the job, the school district will be paying your claim through our third party administrator (TPA). Your claim will not be paid by the State Fund. The benefits we pay are identical to the State Fund. The Kent School District is Self Insured for all on the job injuries. Successfully prepare all students for their future

On-The-Job Injuries If you are injured at work you must report the injury to your supervisor immediately. If your supervisor is not immediately available the injury must be reported before the end of the next work day. The Kent School District is fined if we do not get payment to injured worker within 14 days. Successfully prepare all students for their future

On-The-Job Injuries After you report the injury, pick up a claim packet containing claim forms from your site or department head secretary. If they do not have one available contact KSD Risk Management. Make sure employees know where to get a packet. Successfully prepare all students for their future

L & I Claim Packet The claim packet has two sections. One section has claim forms and information for the employee. The other section has information and forms needed by the employee’s medical provider. Successfully prepare all students for their future

L & I Claim Form In order to file a claim, the top 2/3s of the SIF-2 form must be filled in and signed in two places by the employee. The bottom 1/3 of this form is filled in by Risk Management. Make sure the employee signs the form in the two spots in the lower right hand corner of the center section. Also make sure the employees Social Security number is filled in. L & I Claim Packet If you will be off of work due to your on the job injury, your medical provider must provide a note stating the dates you will be off and a description of the injury. The Activity Prescription Form (APF) should also be completed by the medical provider. A copy of each should be given to your supervisor and to Risk Management. Lost time payments start from the date of an acceptable note from a medical provider. Notes cannot be back dated. Successfully prepare all students for their future

L & I Payments L&I requires us to pay medical bills that are a result of an on the job injury. Please read the Guide to Industrial Insurance Benefits included in the claim packet for specifics about coverage. If you miss work, L&I requires us to pay a portion of your lost income. The percentage depends on you marital and dependent status. The first three days of any claim are not paid unless you are off on the 14th day following your injury. You can use sick leave for these three days. Risk Management needs to be notified immediately when someone is off of work due to an on the job injury. L & I Payments The Kent School District allows you to use accumulated leave to make up the difference between the percentage paid to you as a result of the claim and your normal pay. Risk Management will ask you if you want to use accumulated leave, and will submit an adjustment to payroll to adjust your warrant once a month. Leave can only be used to make up difference when on time loss. It cannot be used if working light duty part time. Successfully prepare all students for their future

Hazard Reporting Every employee has the right and responsibility to report any unsafe act, condition, or procedure they encounter Every employee has the right to make these reports without fear of reprisal Successfully prepare all students for their future

15 Reporting Unsafe Practices or Conditions
Any unsafe practice or condition you identify should be reported immediately to your supervisor. If the condition is in need of repair, your supervisor should submit a work order to maintenance. Unsafe practices or conditions can also be reported to a representative from your site safety committee or to Risk Management if your supervisor is unavailable or unable to correct the situation. If the unsafe practice or condition is life threatening, contact your supervisor, site administrator, and Risk Management immediately. Work orders that are safety related are given higher priority. Emergencies Your site administrator will review the site specific emergency plan with you. This plan includes procedures for fire and earthquake evacuations, as well as guidelines for handling other emergency situations. Your emergency plans must be explained to all employees. Safety Committees Each site has a safety committee. Safety committees meet monthly Safety committees are composed of employer and employee elected members. The number of employer elected members cannot exceed the number of employee elected members. The Chairperson is elected by committee. Successfully prepare all students for their future

18 Accident Prevention Program
L&I requires KSD to have an Accident Prevention Program that is tailored to the needs of our operations and types of hazards involved. “It must be supervised and enforced in a manner which is effective in practice.” L&I requires documentation showing that we enforce safety rules. 19 Accident Prevention Program
KSD has a comprehensive written accident prevention program. The Employee Safety and Health handbook is available at all sites, and on our website at under Safety and Health Policy Yearly training is required by L&I on several topics. These include Hazard Communications, Blood Borne Pathogens Exposure Control, and other job specific training programs. Successfully prepare all students for their future

20 Accident Prevention Program
Site administrators or department heads are responsible for providing site or department specific training. This training should include review of emergency plans, location of first aid supplies and safety bulletin board, safety rules, required personal protective equipment and any other department required training. This may include lock-out tag-out, confined space entry, fall protection and others based on department needs. An employee orientation checklist is available at under employee forms. 21 Hazardous Substance Information
“Workers Right to Know” Standard Material Safety Data Sheets available by phone and fax at: The MSDS information includes hazards of the product, personal protective equipment and precautions required to safely use the product. KSD does not keep a physical inventory of MSDS sheets. All MSDS information can be obtained by calling They will provide the information requested over the phone or will fax it to you immediately. Have the product name, chemical name, or upc code and your fax number available when you call. Successfully prepare all students for their future

22 Operation of Machinery
L&I requires that all employees will be trained for competency on all equipment prior to authorization to operate Successfully prepare all students for their future

Safety Rules Follow all safety procedures outlined in training Personal Protective Equipment will be worn if tasks require it Use appropriate equipment. No standing on chairs or desks Horseplay will not be tolerated If driving or a passenger on district business, seatbelts are to be worn as designed by manufacture. 24 Employee Responsibilities
Attend initial safety orientation at your site Know and comply with safety rules and procedures If you do not know how to do a task safely, ask for additional training Cooperate with co-workers, supervisors, and the safety committee to eliminate accidents Identify and report potential hazards Immediately report all accidents to supervisor regardless of severity Successfully prepare all students for their future

Complete an accident report and submit to supervisor within 24 hours Serve on safety committee when elected or selected Maintain all PPE in a safe and usable condition and use it when needed Participate in ongoing safety training Perform all tasks so as not to endanger yourself or co-workers (Last Slide) Successfully prepare all students for their future
