2 Tips This guide is to help you understand the forms necessary to apply for Disability Benefits through VAC. If a Veteran wishes to pursue an application, it is best practice to direct them to the Command Service Officer for representation. If they prefer to go directly through VAC, or simply need someone to help them understand the forms, you may feel free to assist them in the completion of the application. The application form changes regularly, but generally requires the same information as previous versions.

3 Purpose The purpose of the Application for Disability Benefits is to establish the following: The personal/family information of the applicant; The condition being claimed; How this condition was caused or significantly aggravated by military service; and How it affects the applicant’s quality of life

5 The Application Claimed condition The next several pages comprise the actual application. The number of pages is dependent upon the number of claimed conditions. How is the condition related to service… Quality of Life Questionnaire Signature

6 The Consent Form The next few pages are just instructions (don’t worry about them). The last page is the Consent for VAC to Collect Information from Third Parties form. Typically, it is good practice to name the primary care physician here. Permission for Complete file or only specific info Authorized Parties Signature
