Are you satisfied with the resources available for patients/caregivers? What limitations do you face in terms of receiving support services for yourself? What is the one thing you believe everybody in the community should understand about dementia? How do you like to receive information on Alzheimer’s and/or dementia? That there is no cure. Caregiver Counseling, Support Groups and Services J: All in group have been in touch with the Alzheimer’s Association (AA) which also serves as a case management resource. Education and Training for Family Caregivers J: Members generally wanted more knowledge of disease, steps/stages. Participants appear to have numerous family members but not family support/understanding/involvement. Discussion focused on methods to incorporate family and increase education. Many in favor of whole family case management/education/counseling.

Caregiver Counseling, Support Groups and Services 6: learned to answer the question being asked, don’t elaborate, and just realize that I can challenge him all he wants to, but it will just frustrate the both of us. Awareness 3: nobody wants to admit that something is wrong with them. Health Care: Staff Training 1: doctors should be more engaged. Doctors do not stress information enough to the patient.

Awareness 3: I came from a generation that we did not discuss these things. My father was very secretive about medical issues. I personally believe these issues need to be on the table and discussed. We need a judgment free zone. It’s a lack of education and knowledge. Caregiver Counseling, Support Groups and Services 5: Augusts Service. RSVP. Hospice. Church, Alzheimer's group does not meet anymore. Education and Training for Family Caregivers 7: Sometimes we read things to hear things we want to hear. I see fliers, etc. I just keep learning to keep up.

5/5/2017 Next Steps: ?
33
Next Steps: Prioritizing Goals
5/5/2017 Next Steps: Prioritizing Goals Create a list of priority goals generated from community engagement process. Next to each goal, list possible responses/actions/programs the community might consider to take action. At next meeting, community members will self-select into goal areas, review possible actions listed, and pic their top 1-2 actions using the prioritization criteria (handout). Bring everyone back together to report on the top 1-2 actions for the goal areas. Then have the full group identify 2-3 that “rise to top” again using the prioritization criteria (handout). continue...
34
Next Steps: Prioritizing Goals
The highest priority actions agreed upon by the group will be the foundational to a work plan and budget used to seek funding. Your community might take on more than one priority action, depending on size, scope, and resources.
35
Goal Area Small Group Discussions
Review the priority goal area. Review and discuss possible actions and resources listed for your priority area. Discuss additional action and resources. Add selections of additional actions and resources to the flip chart. Using the prioritization criteria, identify the top 1-2 priority actions that your group feels best meet the criteria; select someone in your group to present them to the full group.
36
Prioritization Criteria
The idea seems doable (we have the right people and resources to make it work in a year or less) It is a good fit for the Southern Nevada community It would make a meaningful difference for the community It is affordable (can be done on a limited budget)
