RRT Activation: No Type and amount of product spilled: Diesel Fuel, Propane Tanks, Various Household HAZMAT Cause of spill: Vessel Grounding Date of spill: 08JUN17 – 09JUN17 Responsible Party: Larry Pettit; Markel American Insurance Key operational activities: Containment, Clean Up, Disposal, Disposition of personal use amounts of marijuana, Vessel Destruction/Removal Major lessons learned: Aggressive response due to grounding on Cannon Beach near Haystack Rock Marine Preserve, High public traffic area Lead Coordinator Contact Information: MST1 Kyle Owens (503) On 08JUN17 at 0446 STA Tillamook Bay had to cease towing operations of the S/V EL CORAZON after recovering both occupants during SAR off of Tillamook Head. The S/V drifted and grounded at Cannon Beach, OR roughly 500 yards South of Sea Lion Rock in the Haystack Rock Marine Preserve. SCR IMD deployed to scene and the decision was made to open funding to remove the threat of oil discharge. BOA Cowlitz Clean Sweep was contracted to remove the fuel and hazardous materials on board the vessel during low tide window on 09JUN17. On 09JUN17 at 0830T, IMD Astoria arrived O/S with the S/V EL CORAZON. Cowlitz Clean Sweep conducted removal of all hazmat and fuel onboard the vessel. IMD Astoria discovered personal use amounts of medical marijuana onboard the vessel. Local PD handled disposition of the drugs. At 1030T, IMD Astoria concluded removal all known pollutants from the S/V EL CORAZON, to include 25gal diesel, 2 propane tanks, and various household HAZMAT. IMD has no further tasking, but will continue to monitor. Pending owner contracted salvage. Over the course of the next few weeks IMD monitored the situation and worked with OR State Parks, vessel owner and insurance company to develop a vessel removal plan. It was determined the vessel could not be safely refloated or hauled off the beach due to extensive damage and limited access. The decision was made by the owner and insurance company that the vessel was a total loss and on 27JUN17 local contractors were used to destroy the vessel in place, clean up and remove all debris. 9/12/2018 FOSC Reports

Type and amount of product spilled: Diesel, Lube and Hydraulic Oils Cause of spill: Vessel Sinking Date of spill: 05JUL17 Responsible Party: Donald Young; Last reported owner Key operational activities: Dive Ops; Boom Deployment; Fuel Removal; Surface Cleaning Ops; Disposal Major lessons learned: No contact with RP during entire case. Lead Coordinator Contact Information: MST1 Kyle M. Owens (503) At 2258T on 04 Jul, D13 requested SCR assistance to locate an EPIRB signal being received from the Aberdeen/Hoquiam area of Grays Harbor registered to 46 ft CFV DONNA (ON: ). At 0039T, STA GH shore party reported they located the EPIRB and found the F/V DONNA submerged and discharging oil creating a sheen on the Hoquiam River, a US Navigable Waterway. Multiple attempts were made to contact the owner of the vessel with negative results. SCR IMD opened the OSLTF and contracted with Global Diving and Salvage to mitigate the pollution threat and conduct clean up. At 0800 on 05JUL17, IMD deployed to scene and directed contracted dive operations and clean up efforts. At 1153T, IMD AST reported that 185 GAL of oily water mix was recovered from all accessible tanks, there is no active sheening being observed and surface cleaning operations with sorbent pads was concluded. Remaining sheen is unrecoverable and will dissipate naturally. 9/12/2018 FOSC Reports

5 F/V Western Skies Sinking 16 Jul
RRT Activation: No Type and amount of product spilled: Diesel Fuel, Lube and Hydraulic Oils Cause of spill: Vessel Sinking Date of spill: 16JUL17 Responsible Party: Derelict Vessel; No known RP Key operational activities: Dive Ops; Boom Deployment; Fuel Removal; Surface Cleaning Ops; Disposal Major lessons learned: Directing contractors to bring demo tools in case of need to break into wood hull to look for oil trapped inside Lead Coordinator Contact Information: MSTC Bradley Bennett (503) At 0530T on 16 JUL, SCC received a report from Astoria Dispatch of the 43 FT CFV WESTERN SKIES (ON ) partially submerged at Warrenton Marina. Astoria Dispatch reported incident to the NRC. IMD AST attempted to follow up with the original reporting source and with the owner with no response from either party. The CFV is actively sheening an unknown amount of diesel so the Warrenton Marina is placing boom to contain the spill. IMD AST has opened a $50,000 fund for clean up efforts. Max potential pollution is still unknown. At 0830T, IMD Astoria advised ceiling for cleanup established at $40,000. Global Diving and Salvage has been contracted and expected to be on scene between IMD confirmed Warrenton Marina had deployed boom around the partially sunken vessel. At 1340T Global Diving and Salvage staged on scene. At 1549T dive operations commenced. At 1920T, IMD Astoria reported Global Diving had completed exploration of all fill ports and removed some product from midship tank. All the tanks now appear to be empty, however, some fuel is trapped in the hull and seeping out through planks at this time. Boom is still in place and absorbent pads are in use but recovery is proving difficult. Dive operations expected to continue until 2200T. At the conclusion today, IMD will try to estimate the amount of recovery. At 2137T on 16 Jul, IMD Astoria advised Global Diving and Salvage removed an estimated 1,000 gallons of water/oil mixture, confident they explored all accessible tanks, and removed all fuel possible. CFV WESTERN SKIES is no longer discharging and environmental team is switching out all sorbent materials. IMD Astoria will meet environmental team again tomorrow on scene to change out the pads and determine necessary future work. 9/12/2018 FOSC Reports

6 F/V Sea Lion II Grounding 22 Jul
RRT Activation: no Type and amount of product spilled: Grounded in Waldport, OR. Potential of 180 gallons of diesel. Cause of spill: Master fell asleep at helm. Date of spill: 22 July 2017 Responsible Party: Carey Stoneburg Key operational activities: NWFF hired to pump out fuel tank. Owner deconstructed vessel and removed it from beach. Major lessons learned: Notice of Federal Assumption was issued to initiate a rapid response. After the insurance company verified, the owner assumed the cost and the remainder of the clean up. Lead Coordinator Contact Information: MST2 Alisha Carr (503) , Add Picture SITREP 1 At 0433T, SNB received notification from CFV SEA LION II (239745) was soft aground just north of Alsea Bay with 01 POB. Master stated he fell asleep but there was no damage to the vessel and he is comfortable waiting for high tide around 1300T. Master reports 29 salmon on board and 100 gallons of diesel. SNB established a COMSKED and is monitoring the situation. IO, CFV, and DEO notified. PENDS At 1130T, Sector North Bend provided update for the CFV SEA LION II aground just north of Alsea Bay with 01 POB. The vessel's master's intentions was to refloat with the incoming tide, however the master cut his anchor line with the incoming tide. The incoming tide has pushed the 34'ft CFV further onto the beach. Sector Columbia River briefed IMD Portland for potential salvage action. DDO North Bend and CFV notified. PENDS. At 1355T, two members from IMD PDX deployed to Waldport, OR to assess the vessel's status and determine CG involvement in salvage and pollution response, if any. ETA is 1730T with an anticipated RON. PENDS. At 1640T, IMD PDX reported they spoke with the Captain of the CFV SEA LION II, who advised them that the vessel was breaking up on the beach, and the fuel had not yet been removed. IMD was authorized to open the Fund for contractors, if needed, and lodging with a ceiling of $25,000. Briefed Response, COTP, D13. IMD PDX is APPROX 30 minutes from scene. PENDS. At 1932T, IMD PDX on scene with CFV SEA LION II. The vessel is high and dry on the sand completely out of the water and has a crack in the stern near the propeller. IMD PDX has hired NWFF Environmental out of Philomath, OR for assistance in the removal of the diesel fuel on board; ETA 2.5 hours. Marine Surveyor for the company is on scene; the owner and captain have no salvage plan worked out at this time. IMD reported that a park ranger is also on scene. PENDS At 2037T, IMD PDX departed CFV SEA LION II for the evening. Due to high tide, NWFF Environmental will be unable to remove the fuel from the vessel tonight. NWFF Environmental and IMD PDX will return at 0530T 23JUL. PENDS At 0700T 23 JUL, IMD PDX was on scene with the CFV SEA LION II and reported the vessel is still in one piece, has not moved, and that there is no discharge on the beach ATT. NWFF Environmental has started pumping the fuel off of the vessel and IMD will update SCC when they are done. PENDS At 0745T, IMD PDX reported NWFF Environmental completed pumping off approximately 180 gallons of diesel from the CFV SEA LION II. IMD PDX is RTB while the owner arranges salvage. Engaged with the Property owner, Police Department and Quinault Tribe. Precarious location for vessels. Lead Coordinator Contact Information: MST2 Mark Campanale (503) At 1613T 26JUL, SCR received a NRC report for a 42' CFV PERWYN (259841) sunk and discharging marine diesel. The vessel sank at the mooring at 220 Monroe St in Hoquiam, WA in the Hoquiam River. The property owner stated he saw the vessel owner depart earlier in the day and had not returned. There was a maximum potential of 800gl of marine diesel. A 20'x20' red sheen had been observed bubbling from the cabin of the submerged vessel and discharging into the Hoquaim River, a navigable waterway of the United States. IMD Astoria opened the fund with a $50,000 ceiling and contracted Global Diving and Salvage. Contractors placed containment boom around the vessel. At JUL, IMD Astoria arrived on scene to supervise the removal of diesel from the F/V PERWYN. Both port and starboard tanks were pumped out and a total of 200 gallons of diesel were removed along with a large amount of oiled pads and debris. Ops were completed at 1000. 9/12/2018 FOSC Reports

8 F/V Pacific Belle Discharge 18 Aug

Shutting off automatic bilge pump. Routine Case with no extenuating circumstances. MST2 Alisha Carr (503) , On 16AUG17 at 1242T, SCR received NRC report for a discharge of an unknown amount of fuel/bilge water from CFV PACIFIC BELLE (OR100ADF) at the dock in Charleston Harbor OR (Coos Bay) due to a fuel line leak allowing diesel to drip into the vessel bilge. Bilge pump was secured and fuel line shut off. Boom was placed around the discharge. SCR briefed Portland IMD. IMD will continue to monitor but did not deploy. CLOSED Marina deployed boom and pads. NOV Issued. Operator stated a cup of diesel would pump out whenever automatic bilge pump kicked on, and would use manual pump to prevent further discharge. Operator made arrangements to take to drydock for repairs. 9/12/2018 FOSC Reports

9 M/V Star Hidra Discharge 07 Sep
RRT Activation: No Type and amount of product spilled: Lubricating Oil, crew needed to add approximately 20 liters of oil to the closed system the every few months. Cause of spill: Leak from the rope guard where it meets the hub of the stern thruster. Date of spill: 07Sept17 Responsible Party: Grieg Shipping II AS (Norwegian) Key operational activities: PSC exam found discrepancies in their oil record log books; dive operations, temporary epoxy repair; administrative order to ensure permanent repair. Major lessons learned: Report of mystery sheen that led state and CG investigators to the STAR HIDRA as the possible source. This case showed good use of departmental interaction to verify the STAR HIDRA as the source and require permanent repair. Lead Coordinator Contact Information: MST2 Rory Jenkins (503) Received NRC report for sheen on Columbia River, suspected to be coming from the STAR HIDRA. Traces of oil were observed on water surface in immediate vicinity of ship’s stern. Global Diving and Salvage examined the stern thruster, which the Port Security Control team believed to be the point of discarge. Underwater survey determined source of oil was from two small leaks on stern thruster hub where rope guard meets the hub. Splash zone was put around the leaking area. Valves were also closed to prevent any oil from going into the stern thruster. Master advised not to use thruster until permanent repairs made. Class Survey recommended to carry out permanent repairs within two months. CG placed Admin order on vessel directing it not to use the stern thruster until permanently repaired. Ship’s engineering log book noted that oil was being added to the vessel’s stern tube tank and stern thruster head tank at an abnormal rate. Chief Engineer could not account for loss of oil from the head tanks. NOV issued. 9/12/2018 FOSC Reports
