2. Creating a form
Step 1
Select the “Forms” tab then select the green “Create a Form”

After selecting the green “Create a Form” button, it should bring you to the Form Builder

Give your form a name and provide a brief description of the form
Select create a page at the bottom of the screen

Select “Add Question” or “Add Text Block” to add your questions to the form

Allow you to fill in your own response to the questions

Multi-Line Questions allows you to insert muli- line questions into your form
You can also provide additional instructions and require them to answer the question

Allows you to select multiple answers, or only one answer to the question

10. Create a Form Uploading a Document
Here, you can upload a document to your form to be reviewed
The maximum file size is 20MB

11. Create a Form Text Block Editor
This option allows you to insert content that will provide additional information to the people filling out the form

Under the “settings” tab, you will be able to control who is able to view the form

Here, you are able to limit the number of submissions
You are also able to limit the time period that submissions can be accepted

14. Create a Form Custom Messaging
Here, you are able to send custom messages to the users when they submit the form, approve the form, deny or when you reopen the form

15. Create a Form Sharing your form
After you have published your form, you can share your form via Facebook, Twitter and can also generate a QRcode
Or, you can simply copy and paste the link under “Share this URL”

16. Create a Form
Now you have published your form!
