What metrics and methods have you or your partner/collaborator organizations put forth to evaluate progress around these collaborative efforts in addressing health equity issues?

Definition/Identification and Outreach What risk factors do you find have the greatest impact on quality/health outcomes? How would you advise organizations that want to move ahead with efforts to identify/ define vulnerable populations groups with high social determinant of health needs? What would be most helpful to you/your organization(s) regarding better identification and outreach?
