CLUSTER HIGHLIGHTS CHALLENGES
Team work (staff shortages, turn over, investigations have led to programs being short staffed and the programs and staff pulled together to get through this rough time) CS-CT has become its own entity as a CRJ division Manchester, CT program opened on 8/9/13 Coventry, CT program opened on 10/21/13 CHALLENGES CS-CT is a new vendor for the CT Department of Disability Services and has faced many learning curves in all CRJ departments while becoming acclimated. Total of 101 medication incidents and occurrences Simsbury average per client over six months = 4.9; Windsor average per client over six months = 1.5; Manchester average per client over two months = .2; the Overall Average for CS CT is 2.1 per client over six months 58 out of 101 medication incidents and occurrences or 57% are client refusals of psychotropic medications Data obtained from MMRs

KEY MMR RESULTS FOR CS CT
As of September 30, 2013: Client Related: 3/9 or 33.3% of the clients are in Day Programs 3/9 or 33.3% of the clients are in school 3/9 or 33.3% of the clients are in sheltered workshop/job training 4 client incidents involving contraband 7/9 or 78% of clients choosing healthy living opportunities within program Staff Certifications: 43% certified in Medication Administration (Simsbury = 40%; Windsor = 63%; Manchester = 25%) 100% certified in First Aid/CPR/AED and PAC/CPI
