A–C: This is a 35-year-old neuropathic insulin-dependent type I diabetic who complains of pain and instability of his ankle when walking. There is marked destruction of the ankle with severe instability and deformity that is not treatable with bracing. D, E: The ankle is stabilized with an arthrodesis using intramedullary fixation and returns the patient to an ambulatory status requiring a rocker-bottom shoe and an accommodative insert.
