Our History New York State became a national model over time thanks to the dedication of many working toward a shared vision with government The past forty years leave a legacy of community integration in favor of institutions Community services have expanded to serve over 130,000 individuals with developmental disabilities

Multi Year Housing Strategy
Development of new certified residential opportunities Geared to meet the needs of those living at home Developing additional non-certified options through the Integrated Supported Housing program

Integrated and Supportive Employment
28,000 people receive supports to help them work and find employment as well as gain employment skills Person-centered approach to determine individual interests and options for employment Alternatives to sheltered workshops include Pathway to Employment, day habilitation, community pre-voc, or other employment services

People First Care Coordination
The New York State Office for People With Developmental Disabilities is moving forward with plans for a comprehensive and holistic approach to coordinating care called People First Care Coordination, supported by enhanced information technology. People First Care Coordination will be provided through Care Coordination Organizations – also known as Health Homes - that are tailored to people with intellectual and developmental disabilities.

Benefits of People First Care Coordination
Enhanced person-centered planning Cross-systems service coordination Families can obtain more help managing services while still taking on as much of a role as they have in the past Improved information technology A greater focus on performance measurement and incentives

CCO/HH Application Public Outreach
OPWDD and DOH incorporated several changes suggested through the application public comment period: Clarifying individual protections Aligning PCP model with regulation Individual & Family advisory councils Twelve public forums are being held throughout the state to share information and answer questions across the state. Three next week: Queens, Thiells, Tupper Lake

Managed Care Transition
OPWDD is developing a specialized managed care model to serve people with developmental disabilities People First Care Coordination will be the foundation of this system

Five-Year Transition to Managed Care
Implementation of People First Care Coordination Develop specialized Medicaid Managed Care Organizations (MCOs) Voluntary enrollment in specialized MCOs until such time as two or more MCOs are available in each region of the State, and quality & satisfaction have been assessed Mandatory enrollment into MCOs

CCO / HH Implementation Timeline
10/10//17 Draft Application Published June 2017 Submitted State Plan Amendment and 1115 Waiver Application Summer 2017 Final Application Published October 6, 2017 Applications due December 1, 2017 State Review of Applications, Approvals December – February 2018 CCO/HH Readiness Activities February 2018 to June 2018 CCO/HHs Provide People First Care Coordination July 1, 2018

State Plan Amendment and 1115 Transition
OPWDD has submitted a SPA to CMS to amend New York State’s existing Health Home spa to establish DD as a single qualifying condition for DD- specialized CCO/HHs OPWDD is seeking approval to transition authority for services from 1915(c) waiver to the MRT 1115 waiver Public comment period was conducted

Affiliating with a CCO/HH if you have not already Medicaid Service Coordination Transition Get ahead of the curve: Learn about health and behavioral health sectors Disability Network Business Acumen Learning Collaborative efforts will support community-based organizations involved in the switch to CCOs and managed care
