BACKGROUND Purpose: Improving the dental access and assessment for patients. Problem Solving: Try to solve dental resource insufficiency problem and get parents thinking about their children’s teeth. Population: Northside Health Center patients ages one to five. Global Aim: Improving the dental health of children age 1 to 5 years old Previously had no system for tracking dental pain and did not have age specific and insurance specific resources for patient handout.

Standardized documentation for dental pain assessment
AIM AND DRIVERS AIM HIGHLIGHTED DRIVERS To increase the % of patients ages 1 to 5 years who are assessed for abnormal dental pain per parent report and offered dental provider resources at the most recent well child visit from 50% to 90% by 11/22/16. Standardized workflow for assessing dental pain during well child checks Standardized documentation for dental pain assessment

BACKGROUND Many barriers prevent Avondale children and families from thriving and reaching their full potential

AIM AND DRIVERS AIM HIGHLIGHTED DRIVERS
Meet all prioritized basic needs* for three Avondale families** by 11/22/16. *Defined as reaching a score of at least 3 on the Arizona Self-Sufficiency Matrix. **Population: Avondale families with newborns born at Good Samaritan Hospital who are CCHMC Pediatric Primary Care Center patients and were referred to the Legal Aid Society for legal help during pregnancy. Engaged moms willing to accept help for social needs Effective means of communication between families and PPC team

Graduation – Sneak Peak
June 19th – 8:30 AM to 4 PM 10:40 – 11: You will prepare a poster and share in Storyboard session. Posters will be hung around the room, and others from our network All Children Thrive will circulate. Bootcamp teachers will also circulate and look for these items: SMART AIM KDD MEASURE PDSAs WHAT YOU LEARNED & HOW YOU WILL USE TOOLS IN FUTURE Reflections at 11:45 AM with broader group Certificates will be provided to graduates!
