Purpose: Improving the dental access and assessment for patients.
Problem Solving: Try to solve dental resource insufficiency problem and get parents thinking about their children’s teeth.
Population: Northside Health Center patients ages one to five.
Global Aim: Improving the dental health of children age 1 to 5 years old
Previously had no system for tracking dental pain and did not have age specific and insurance specific resources for patient handout.

Meet all prioritized basic needs* for three Avondale families** by 11/22/16.
*Defined as reaching a score of at least 3 on the Arizona Self-Sufficiency Matrix.
**Population: Avondale families with newborns born at Good Samaritan Hospital who are CCHMC Pediatric Primary Care Center patients and were referred to the Legal Aid Society for legal help during pregnancy.
Engaged moms willing to accept help for social needs
Effective means of communication between families and PPC team
