2 HISTORY OF KENDAL AT HOME
Started by Kendal at Oberlin in an effort to serve more older adults in northern Ohio Separately incorporated in 2003 as a part of the Kendal Northern Ohio Affiliate Operations began in April, 2004 First and only program in Ohio to date Modeled after Cadbury Continuing Care at Home (purchased intellectual property)

Started as a traditional lifecare membership program Based on a 1 time entrance fee and monthly fee model Entry Fee based on age and program option selected Average 75 year old ~$50,000 Monthly fee ~ $650

4 CONTINUING CARE AT HOME
Offers a comprehensive package of long term care services including care coordination, home care, assisted living, adult day care and skilled nursing. Applicants must qualify medically and financially for the traditional program

6 10 YEARS Served over 240 members in 10 counties in northeast Ohio since inception No permanent facility placements over the 10 year period Reduced re-hospitalization rates of members to less than 3% in a 30 day period Consistently excellent satisfaction survey history

7 10 YEARS A community has developed through social, wellness, and education events Delivering on the promise to help members remain in their own homes Creating unique service delivery options in order to meet member needs

8 FINANCIAL HISTORY Initial capitalization of $688,000/$518,000 was loans or program related investments Cash flowed itself and grew reserves of over $750,000 after 1 year of operation Membership fill-up was slow but has increased to more than double the average growth over the last 3 years

11 ADDITIONAL BENEFITS Increased revenue to the CCRC through the utilization of ancillary service lines such as wellness programs, meal program, home health. Created opportunity for strategic growth and development of additional programs and concierge type services. Care Plus and Care Management models, Preventive Home Maintenance Program Development of “tandem” contracts for members who may chose to move to IL in the CCRC in the future. Increase Kendal’s brand awareness through an aggressive inbound marketing program.
