1 Primary Care Transition Support Groups “A New Concept” Learning together to understand General Practice

2 PCTSGs Aims Offer a structure for peer support for doctors and practice nurses within their first 2 years of qualification. - Learning together to understand GP within the practice and its wide community role. - Learning about each others roles, strengths and weaknesses to better serve the patients needs.

3 PCTSGs Providing opportunities for joint learning, peer review and support for appraisal and regulatory processes. - CPD - Mandatory - Clinical - Admin

4 PCTSGs Provide opportunities for continuing education for new professionals working in general practice and linked to a CEPN. - New ways of working (24hrs) - New ways of providing care (Pas, Para, Pharma, Nurse) - New service & educational networks Two year pilot programme hopefully leading to wider and smaller groups within localities.

5 PCTSGs Objectives Groups will have a facilitator, effective planning and delivery of learning. Groups will develop own governance & organisational structure. Groups will meet at regular intervals, a minimum of 8 per year, at a suitable venue for group support work, education and training.

6 PCTSGs Objectives Groups may invite guest speakers/presenters for clinical updates or primary care skills training, or may utilise the expertise and experience within the group membership. Groups will respond to the declared learning needs of the membership, formulating a programme of events in response.

7 PCTSGs Objectives groups may act as a forum for shared learning.
  - Significant event analysis - Audits Groups are funded by HEKSS as part of our primary care initiative for greater team working and collaboration between professionals in GP.

PCTSGs Adults learn best when… The learning is perceived to be relevant. Builds on previous learning. They reflect. Identify what they have learnt and what they need to learn. Be involved in the planning of their education. Can learn from role models.
