To enter information students login using: Their SNC network username (first 4 letters of last name, first and middle initials) The password they created for their account Student’s Health History Information regarding past health/mental health conditions, allergies, and medications. Any conditions requiring ongoing care on campus must a provide plan of care from their provider. Health Insurance If you have not submitted other insurance information to St. Norbert College by August 1, 2017, you will automatically be enrolled into the High Deductible Plan (St. Norbert College/ WAICU Plan) (under age 21 is $127/ month 4

16 HEALTH INSURANCE We partner with the American Red Cross for the blood drives. The walking program challenges members of the St. Norbert College community to team up, count their steps and improve their health through walking. CPR/First Aid/AED certification classes: Certification is through MEDIC First Aid and is valid for two years. Training consists of two, three-hour sessions. The fee is $25 The take the challenge wellness program is a seven-week holistic wellness program designed to engage the students, staff and faculty of St. Norbert College in taking ownership of their physical, emotional and spiritual health. Each week will present a new challenge. Participants are encouraged to have a “buddy” for encouragement and support (on campus or at home).  Earn points each week by completing activities that promote healthy eating, exercise, relaxation, and wellness. Our hope is to plant seeds that empower individuals to take steps toward improving their general health/well-being and the health/well-being of those supporting them. The Campus Center offers a variety of wellness classes to help you get or stay in shape. All classes are free to the St. Norbert College community and are held in the Campus Center’s Health and Wellness Room, or gym, unless otherwise noted. The MFFSC is open (academic year): Monday-Friday, 6 a.m.-midnight Saturday and Sunday, 11 a.m.-midnight STUDENT HEALTH 101: SNC Student Health 101 wellness magazine: eating well, managing stress, relationships, workouts and more. Take out your smart phone and Text SNC to 40691 *Students will be automatically enrolled in the high deductible plan of the WPS/WAICU student health insurance if your personal insurance policy is not entered online (your insurance information is the waiver out of the SNC plan) 16

17 MEDICAL WITHDRAWL Withdraw from all courses that semester and withdraws from SNC Receives “W” in all courses No tuition reimbursement Does not negatively affect GPA for coursework not completed Must re-apply to SNC and have an approved plan of care in place A health fee of $125 per semester is assessed every student for general services provided in Health and Wellness ServicesA health fee of $125 per semester is assessed every student for general services provided in Health and Wellness Services including basic counseling services as well as access to the state of that art fitness center/aquatics center Advanced medical provider and diagnostic lab services are fee-for-service and billed to the student’s insurance (deductibles and co-payments apply as per individual insurance policies). Health savings accounts (HSA) and flexible spending accounts (FSA) are eligible for payment of deductibles and co-payments. Unlimited Nursing Services Work under standing orders of a local physician Assess illness or injury and provide basic care to include: surgical follow up Upper respiratory conditions Gastrointestinal conditions Dermatological conditions Emotional and stress-related conditions Perform limited lab screening tests: Strep Mono Urine Influenza and more Assist in developing a plan of care for health promotion and your success Coordinate care with home provider Travel Consultations Health Education Advanced Medical Care Advanced medical provider evaluation and treatment Prescription medication on campus and at local pharmacies Advanced diagnostic lab Physical exams for employment, student teaching, etc. 17
