Key Learning: Look for the green tag before accessing a scaffold
INCIDENT IP was asked by his Supervisor to check the diameter of a flange. To do so, he had to access an incomplete scaffold. The scaffolders were nearly finished but were still securing the ladders to complete the scaffold. After taking the measurement he used a different ladder that was not secured, he fell when he stepped off the scaffold onto the ladder ROOT CAUSES: Poor supervision on site; multiple contractors were on site with uncertain command Using unfinished scaffolding Inadequate supervision Wrong instruction given Uncertain command of overall activities Trainee PDO Supervisor left in charge of high risk site Failure to intervene by all parties Lateral Learning Inadequate management of activities on site Recommended Actions: Discuss this alert in toolbox talks and HSE meetings and Distribute to all contractors Post on HSE notice boards Include in site HSE induction Key Learning: Look for the green tag before accessing a scaffold Swollen Ankle Contact MSE511 for further information or visit HSE website LTI No Alert No February, 2011
