The job was cleaning and sand blasting works within a large temporary habitat around the slugcatcher area. What could go wrong? What could be the hazards to personnel? What could be the consequences?

7 Have you been involved in the risk assessment or informed of risk assessment on similar types of activities?

8 TEMPORARY HABITAT What happened
There was cleaning and blasting on live hydrocarbon containing lines. There was a concern on board regarding the enclosure preventing natural ventilation. This concern was raised through normal company channels but not sufficiently addressed. This escalated to a complaint to the HSEx by a Si971 representative. The HSE Inspectors visited the installation and the complaint was upheld and all work stopped in the habitat. The Safety Rep who first raised their concern was contacted and thanked by the company and also the HSEx.

12 TEMPORARY HABITAT Root Causes:
The large sheeted area that was installed all around slugcatcher module changed the requirement to provide open natural ventilation. An Operational Risk Assessment was not in place to assess the risks associated with the natural ventilation being removed (PFEER requirement) . Lack of understanding of the procedures when cleaning and blasting live hydrocarbon lines. The company did not effectively provide relevant information on the risks to employees. How the issue was resolved: The communication process for managing issues raised by Safety Reps has been reviewed after the breakdown between the offshore management team and Safety Reps. On this particular installation a new process is now in place to manage Safety Critical impairments (ORAs). Also a new procedure is now in place for sand blasting and cleaning on live pipework.
