Ways to get to Google Forms
Type the URL docs.google.com/forms On Google home screen press the 9 boxes(Google Apps) in the top right corner and click on Forms, you may have to click more.

Which sHould I Use? You have the option at the top to choose Blank form or Blank Quiz. Blank Quiz does most of the work for you but you still need to know how to change the settings. So, I will be clicking Blank form and showing you how to turn it into a quiz. It would be the same for both.

First Name your Assessment
Click on Untitled form and type the assessment name.

Adding Questions Click the Plus sign on the right side in the middle of the screen.

Types of Questions Multiple choice are the only questions that Google Forms can grade.

Adding Image, Video, and Sections
Add Questions You can add images on answers as well as in the question. You can add a video and ask questions about it. You can add sections to make certain questions show on the screen at the same time. Add Title and Description Add Image Add Video Add Section

Turning your Form into an assessment
Click Settings at the top In the General tab: You have the option to collect s You can restrict who can take the assessment You can restrict the number of responses You can even restrict if they can see the assessment after it has been submitted

In the Settings Tab: You can show a progress bar You can shuffle the question order but not the sections You can allow for multiple attempts You can leave a message for when students submit the assessment

Settings Tab-Quizzes Here is where you turn your form into an assessment. Click the dot so it is on the right side to make it an assessment You have the option to give students scores immediately or later You also have the option to let students see the questions they missed

Type your question. Add any video or pictures needed.

Type your answers. Add any pictures if needed. If you want Google Forms to grade this question, click Answer Key in the bottom left corner. I alway require my students to answer all the questions the button on the right side will allow this to happen.

Grading Multiple Choice Questions
Add the points possible for the questions at the top by the question. Select the correct answer or answers from the answer choices.

Preview your Assessment
Click the preview button (EYE) at the top of the form. This will open the assessment in a new window and allow you to preview how it will look to anyone else.

Assigning your Assessment
There are a few ways to assign the assessment: Use the send button at the top of the form to the assessment to students (this one takes a little longer but only students that are in class get the ) You could also go to Google Classroom, click the student tab, click the students you want to , click actions at the top,then select . Next, add the link to the form in the and send. Attach the form to an assignment on Google Classroom (everyone can see the assessment even if they are not at school and unless you delete the assignment off Google Classroom, it will remain there for the students to click on later)

Reviewing Student Responses
Click Responses next to Questions at the top of the form. The green sheet is there so you can create a Google Sheet of all the responses. The hamburger menu icon is for more options.

Score Assessment You can use the response page and click question and grade 1 question at a time. (Really Like) Responses to Bridge Quiz You can also use the google sheet that you can create. Excel Sheet Responses to Bridge Quiz

Assigning a Grade If you use the Response page to grade once you have scored them, click on summary and then click release. If you use the spreadsheet, you will have to students or just let them know in class because you cannot enter a grade.
