In response to the County’s FY Strategic Initiatives and the changes to health and public health that are being worked on both Nationally and Statewide we developed the draft HD Strategic Plan What are upstream factors ; factors that result in poor living conditions, lack of employment and education opportuities

Tells us: What we are doing well and what needs improvement Where we need to go in response to community needs How we will get there

Our Process Community involvement of stakeholders
Analysis of county health status Comparison to other county assessments Analysis of current data Comparison to other county assessments 21 community group meetings December 2010 through March 2011 On-line survey Returned to the community with draft strategic plan to get feedback and refined document based on comments. 16 return meetings in the community.

Outcomes Community group meeting attendees were concerned with health challenges: Teen births Obesity Diabetes Healthcare access Violence Drug/alcohol use Accidents Asthma in children Mental health Bullying in schools Dental care Perinatal mortality Make it alphabetical

Our Initiatives 1. Empower the community
to improve health through programs, policies, and activities 2. Enhance community health and safety by emphasizing prevention 3. Ensure access to culturally and linguistically appropriate, customer centered quality services These initiative arose from the assessment of the County-wide results Each initiative has clear objectives to be met within 5 years. Activities will be developed to address each of the initiatives and objectives. Do each of the initiatives resonant with you? 9

Initiatives Initiative 1 : Empower the community to improve health through programs, policies, and activities In 5 years, Monterey County Health Department will increase opportunities for community participation in public health dialogues We heard from the community that they want to be involved in our efforts and they want to support policies that improve the health of themselves and their families . We are continuing to engage with the community in developing the best ways to communicate them. The BOS has considerable experience in engaging the community and we look forward to opportunities work with you in this area.

Goal 1: Foster all residents’ health and well-being Goal 2: Work collaboratively with the community to develop and achieve the general plan’s vision for a healthy community Goal 3: Create convenient and safe opportunities for physical activity for residents of all ages and income levels Example of how we propose to address this initiative based on what we heard from the community and confirmed by the data is to include a health element in the General Plan Monterey County Health Department, Strategic Plan (pgs )

Initiatives Initiative 2: Enhance community health and safety by emphasizing prevention In 5 years, Monterey County Health Department will strengthen the community’s ability to respond to safety issues STRYVE grant 13

14 Example: Striving to Reduce Youth Violence Everywhere (STRYVE) Through Local Public Health Leadership Goal: To prevent youth violence in Salinas, using a public health approach Process: The Public Health Bureau will lead a 5 year planning, implementation and evaluation process that identifies strategies to reduce the risk of youth violence focused on youth, families, and neighborhoods 14

Initiatives Initiative 3: Ensure access to culturally and linguistically appropriate, customer-centered, quality health services In 5 years, Monterey County Health Department will ensure access to health care through culturally and linguistically appropriate customer service and by aligning public health, primary care, behavioral health, and community resources with health care reforms

Collaboration with Natividad Medical Center on improved translation services Extension of service hours at clinics Integration of Behavioral Health clinicians into Health Department Primary Care Clinics

Next Steps: Implementation
Provide training for Health Department staff Prioritize activities relating to the 3 initiatives Develop and/or align activities with initiatives through a collaborative process Involve community Other county departments and services State organizations Other partners Start implementation internally through staff trainings and educational series of discussions. (include cultural sensitivity training needs) Customer Centered care with quality improvement focus. Identify short and long term tasks When we mention State, we can reference CDPH for the recent CTG grant 17

I want to end by stating that its not that we the haven't been working to improve health outcomes but we need to change our approach. Counties across the nation are moving to the upstream approach. The bottom line is we need a cultural shift to improve the health of our diverse community. Certainly we have a lot of work to do. As you see sometimes the solution is evident but we have the make the shift to achieve success. San Diego, CA
