2017 MPUMALANGA OPENCAST FATALITIES
On 30th January 2017 1.An employee was busy unblocking a blockage at the screen feed conveyor belt and the belt roll backwards and pulled him towards tail end and fatally injured him. On 10th April 2017 2. Employees were busy removing the feed chute from the double roll crusher of the crushing plant, the feed chute dislodged and now deceased fell from the walkway and succumbed due to his injuries.

REQUIREMENTS 1. Application letter signed by a Mine Manager 2. Risk assessment by Explosive Expert 3. Plans signed by Mine Manager and Mine Surveyor 4. Letter of consent from affected parties. Note that 2 copies of each must be submitted to the Principal Inspector of Mines.

APPLICATION FOR MINING WITHIN 100m
REQUIREMENTS 1. Application letter signed by a Mine Manager 2. Risk assessment by Rock Engineer 3. Plans signed by Mine Manager and Mine Surveyor 4. Letter of consent from affected parties Note that 3 copies of each must be submitted to the Principal Inspector of Mines

APPLICATION FOR BENCH DELIVERY
REQUIREMENTS 1. Application letter signed by a Mine Manager 2. Standard Operating Procedure (S.O.P) 3. 2 Plans signed by Mine Manager and Mine Surveyor 4. Appointments of the Blaster and Blasting Assistants with Qualification Note that 2 copies of each must be submitted to the Principal Inspector of Mines. NB! PLEASE APPLY 3 MONTHS PRIOR.
