and Funding for this statewide effort is provided by The work I am sharing today is made possible by funding and partnership with UC UCEDD/ Nisonger and DODD We are all part of the Ohio Developmental Disability Network WE are authorized under the DD Act State Councils on Developmental Disabilities Protection and Advocacy systems University Centers for Excellence in Developmental Disabilities Education, Research and Service Projects of National Significance We all work collaboratively to support independence, advocacy and community integration of individuals with developmental disabilities through: Interdisciplinary training of students and professionals on developmental disabilities Research and analysis to develop new knowledge on disability that can be applied to programs and policies’ Information sharing and technical assistance for the community at large Community Education and Collaboration

What is the LifeCourse Framework
The LifeCourse Framework was created by families to help individuals and families of all abilities and all ages develop a vision for a good life, think about what they need to know and do, identify how to find or develop supports, and discover what it takes to live the lives they want to live. Introduction Activity Read Slide out loud, Typically try not to read slides verbatim, but… Read slowly to yourself again, what phrase stands out to you and why? Turn to the person next to you, introduce yourself, what brought you here and what stood out and why.

What is your vision of a Good Life
TRAJECTORY When did you begin dreaming about what your life would look like. What were the first memories you have of picturing your future? Where you 2 or three when you imagined being a parent? Were you a teen when you pursued your first job and began to imagine growing in your skills and making more money? Was it when you were asked by a teacher or parent what do you want to be when you grow up? It is unfortunate but true that once faced with the reality of disability we often temper or change these conversations for ourselves or family members. So as a group at your table I want you to take the large Paper with the trajectory on it. And I want you to briefly discuss amongst your elves what a good life looks like. For this exercise I want you to look at it over all and not limit it to how your life is currently but just over all what is important for a GOOD LIFE

And when you look at what isn’t part of what you consider a good life, can you identify a few experiences that would lead to that out come? Trajectory towards Life Outcomes Friends, family, self-determination, community living, social capital and economic sufficiency Focus on Past, Present and Future Life Experiences at any Life Stage Trajectory towards Life Outcomes Vision of What I Don’t Want So what these tools do is enable and encourage us to look at Past Present and possible future Life experiences that empower us to identify and pursue a trajectory towards the desired outcome or good life. Daily Life and Employment What do I want to do all day? Healthy Living What do I need to do to stay healthy (physical and mental) so I can have the live I want? Community Living Where do I want to live and what support do I need to live there? Safety and Security What do I need to be safe (emergency plan, support in my decisions) or Secure (financially, phsycally) BLUE BOOK: Another piece of the framework are the 6 life Domains. When they looked closely at individuals and families who had successfully integrated services to create their good life they did not just do this in one area. All of us develop our lives across different areas. Everyone (whether they have a disability or not has to figure out what they are going to do during the day…school, work, volunteer. Where they are going to live and how they are going to stay safe and healthy. Using the CTLC tools helps you evaluate any of these domains at any age and identify next steps or experiences that will lead you on your trajectory to a good life. Social and Spirituality Who do I want to spend time with and what things will I do with them? Citizenship and Advocacy (valued roles, Able to make choices, setting goals, responsibility, leadership, peer support)

A One (1) Page Description can be used
For a specific purpose: for example new situations- like a new job; meeting new people; at the front of the persons records. An at a glance positive way to share key information about: What people like and admire What is most important to How to best support Purpose: to briefly outline the most likely possible uses for a one page description and the content that is core for an effective one page description Script: A one page description is developed for a specific purpose. A one page description does not take the place of a person centered description.

Like and Admire What people like and admire about you gives you value. It is important to be able to identify these because Asking other what they like and admire in me helps me see my own value If I can identify my value, I can better be of value to others With Value comes confidence With Confidence comes bravery With value comes relationship Meaningful relationship prevents isolation PURPOSE: Define Important TO Six categories that commonly make people happy, fulfilled, content, satisfied, comforted. (other synonyms: pleased, at ease, comfortable) SCRIPT Emphasize that this is about humans not just about disability – Ask how many people are living with another adult How do they know if the person they live with is happy? What do they do? Select 1 person and ask her (him) if her partner’s behavior is ever at odds with what he says (his words) Ask “when what he says is different from what he does – which do you listen to, words or deeds?” Hopefully the person will answer – “deeds” – so we already know that we should listen to behavior Note that when we do not trust people we are hyper-vigilant about what they do much more than about what they say For people who use disability services - We often need to look at behavior because we have done a really good job of training people to tell us what we want to hear When people don’t use words to talk, we have to listen to their behavior TIPS: TIME: 2 minutes NOTES: TLC-PCP
