11 YH - Evolving Workforce Configurations
1.-This is the historical configuration of the GP workforce 4:2:1-Have you ever tried balancing a toblerone like this? ***** It falls over. This is what is starting to happen to some General Practices -The key to workforce stability in General Practice is to change the shape of workforce configurations That means more GPNs working at all levels, many more health care support workers, and a broader skillmix embracing allied health professionals-from pharmacist to Physicians associates,ACPs, care navigators, nurse assocoates and medical assistants. 2.Moving towards this-or even 3.This -Accelerating this shift is what we are about with the ATPS (Next slide)

Entry into general practice – raising the profile as a first career choice and increasing the availability of training placements for students Establishing the role of the GP Nurse – ensuring appropriate training and support is available for new GPNs Enhancing the role with professional development and career progression Expanding the healthcare support workforce with standardised training and career paths.

14 It’s a call to action! -It describes the need to increase numbers of GPNs and secure their career pathway and development at all levels -It calls for a rapid growth in GPN Educators and leaders -It suggests that every GPN should become a mentor -

-To GPNs like you it states the obvious -We want General Practice Nursing to be a top job destination offering a lifelong rewarding career

16 The Advanced Training Practice Scheme across Yorkshire and the Humber
“Grow Your Own” Its amazing what you can achieve with 80p! -We want to tell you about the ATP-the Advanced training practice scheme in Yorkshire and the Humber. We began as a pilot in 2009-well in advance of the Forward View, the 10 point plan and the GP Forward View. My local colleagues saw a looming workforce crisis in General Practice and began taking steps to understand our workforce better and develop a network of General Practice based hubs to support workforce development. -We are sometimes known as the 80p! Because we are such good value for money…we are from Yorkshire and like everything under a pound! -Why is this important? (next slide)

17 ATPS Idea Industrialise Normalise
Eight training hubs based in general practice covering three LWABs, 23 CCGs, 794 practices There is a shared vision and understanding of workforce crisis which is informed by Y&H Workforce Tool for primary care There has been sustained funding since commencement in 2009 We are ultimately ‘change managers’ for the workforce in primary care Idea Industrialise Normalise Change management in a very stressed GP setting isn't easy. Our 8 hubs have full regional coverage and have worked really hard to engage at every level with a compelling workforce narrative underpinned by robust local workforce data. We have been able to achieve this through the HEE YH commitment to year on year funding. The ATPS is now embedded as part of the fixtures and fittings of our regional Workforce plans that inform the LWABs and 3 STPs in our region. Our journey from idea back in 2009 to growth and industrialisation and now normalisation has been achieved. And-We ARE making a difference!! (next slide)

ATPS- Achievements Nearly 30% of all practices in YH now take regular student nurse placements GPN Ready and HCA Apprenticeship schemes embedded and developing further Changes in employing behaviours e.g.  no. FT GPN HSJ Award Winners Workforce Category 452 students placements over 15,344 days 48 new sign-off mentors 75 HCAs & 56 GPNs created 4%  in GPNs <35 yrs old -the more nurses that have student placements the more they want to consider careers in General Practice- -our GPN Ready and HCA Apprenticeship schemes are in their 2nd and 3rd years respectively. (HCA = 66) We are increasing our conversion rate from student nurse to newly qualified nurses being employed directly by General Practices. The 8-12% increase in GPNs under 35 years old is a 50% increase in headcount and the majority of new entrants at this level are full-time with long service careers in front of them. -Our strapline is –”Grow your own”-employ, train and retain your staff So we can now demonstrate changing the shape of workforce configurations in General Practice in Yorkshire and the Humber

Cultural Shift – dispelled the myth that new practice nurses need post qualification secondary care experience-they don’t! Massive career choice sea change-32% pre-post to 91% after would consider GPN career Value added holistic care experience for nurses pursuing acute sector careers Identifying and promoting GPN educators and leaders Supporting Return to practice programmes
