New name for West Virginia Health Transformation Accelerator called for in section 5 of the SIM plan An independent 501(c)3 with a three member governing board Founded in September 2016 and operational since January 2017 Gary Murdock is board vice president The presidential election, shift away from the Affordable Care Act necessitated a change in focus Less focus on implementing alternative payment models—more on aiding existing value-based, innovative health care projects

Accountable Health Communities (CMMI) Partners In Health Network, Inc. selected for one of 12 cooperative agreements nationally Providing logistical support, aid in securing MOUs/DUAs for data sharing with DHHR Marshall University Community Health Worker projects led by Drs. Richard Crespo and Dick Wittberg Consulting on creating sustainability by engaging insurers in reimbursement discussions Bringing Center for Medicare and Medicaid Innovation (CMMI) to the table to discuss role of Medicare Coordinated a Health Impact Assessment Workshop with Benedum and Greater Kanawha Valley Foundations and Pew Charitable Trusts – Health Impact Project Projects WVPHI assisted with this year

5 Where the WVPHI is September WVPHI received $100,000 grant from the Claude Worthington Benedum Foundation Grant will assist with startup costs and meeting certain deliverables September to November WVPHI’s contracted resource served as a Jonas Salk Fellowship through the Jewish Healthcare Foundation Fellowship allowed WVPHI to make connections to further bolster Community Health Worker sustainability

Continue supporting the Community Health Worker project, as long as requested Disburse mini-grants of up to $5,000 to kick- start value-based projects (~$10,000 available in total) Partner with DHHR and federal agencies, such as Appalachian Regional Commission and Office of the National Coordinator, to host a Health Information Technology Summit (1Q or 2Q of 2018) Assist in provider education and outreach on Project ECHO in the state (Ongoing) Convene ad hoc meetings of ECHO clinic organizers to share best practices and lessons learned ACHIEVE SUSTAINABILITY AND A REPRESENTATIVE BOARD OF DIRECTORS
