1 Lesson Four: Viewing a Patient’s Chart
At the end of this lesson, you will be able to: Access the tabs within the patient’s chart Understand the types of information displayed on the various chart tabs

2 Lesson Four: Viewing a Patient’s Chart
To open a patient’s chart, double click their name on the Patient Access list. To go to a specific section of the chart, click on the Chart button on the toolbar, and select the target section of the chart. You can also search for a patient that is not on your list by using the previously discussed search methods.

3 The PowerChart Patient Chart provides quick and easy access to specific patient information. Patient demographics, allergies, visit history, current orders and results are visible from the Patient Chart.

4 The Patient Demographics Bar provides information such as patient name, age, DOB, sex, Location, Medical Record Number, and Financial Number. It also contains allergies, isolation precautions and the patient’s clinical weight used by the Pharmacy. Hyperlinks, indicated by an underline, can be accessed by a single click of the mouse and will take the user to additional information.

5 The Refresh button is used to ensure that you are viewing the most up-to-date patient information. The ‘minutes ago’ indicates the last time the displayed information was refreshed. Click the button to update the information.

Let’s take a look at some of the available chart tabs within PowerChart. Keep in mind that many options are security-driven, so all displayed options may not be available to you.

The Chart Tabs within PowerChart appear on the left side of the screen in a collapsible menu.

You can elect to hide the Chart Tabs that display on the left side of the screen by pressing the pushpin icon which collapses the menu. By default, the icon appears pointing downward. This indicates that the menu will display in a normal manner on the left side of the screen.

9 Lesson Four: iView/I&O Tab
The iView / I&O tab is the section of the chart that is used to enter and view various types of results that would be entered on a flowsheet. In iView those with security can: Document View Modify Unchart Only those with security will be able to document on the iView (Interactive) flowsheet. Student documentation will need to be authenticated. Documenting in iView is discussed later in this tutorial. iView/I&O Tab

The Orders Tab is used to view orders and medication information. Also, those with the required training and security can enter orders from the Orders Tab. There are two main sections of the Orders tab: the Clinical Categories Navigator and the Existing Orders Profile. The Navigator lists clinical categories. From the Navigator, you can select the categories from which you want results displayed by checking / un-checking the applicable box. The right side of the Orders Tab is the Order Profile. The Order Profile lists patient orders and medications, as well as their statues and detailed information. ORDERS TAB

You can filter the types of orders you want to display by clicking on the downward triangle in the filter window and selecting from the available choices. In addition, you can click on the ellipsis for more filtering options. ORDERS TAB

You can view additional detail about an order by right-clicking the order and selecting the appropriate option. The selection options for viewing information are Order Information, Comments, Results, Reference Information and Print. ORDERS TAB

The Form Browser tab window displays a directory tree of completed forms for the selected patient. You can open a form to view the information by double-clicking. Those with security access can also edit forms by right clicking and selecting the appropriate function. FORM BROWSER TAB

The Inpatient Summary Tab pulls information from different areas of the chart into a single convenient view. The Inpatient Summary contains a quick view of information such as problems documented in the Problem List, Measurements and Weight, Diagnoses, Allergies, Vital Signs and Intake and Output. Click the hyperlinks to display results for the last 10 days. INPATIENT SUMMARY TAB

The Results Review Tab contains flowsheets of documented result information. The Recent Results sub-tab contains two major sections: The left side is the Navigator and the right side is the Results Display. By selecting a category in the Navigator, the Results Display will show applicable results. Navigator Results Display RESULTS REVIEW TAB

From the Patient Care sub tab you can view Patient Care results that have been documented in a PowerForm or iView. RESULTS REVIEW TAB

Changing the View You can also change the view to Table, Group, or List, depending on how you want results displayed. Simply choose the appropriate option. The Table View is the standard flowsheet viewing method, but you can also choose the Group View or the List View. RESULTS REVIEW TAB

Results Abnormal results, for example high or low values, will be specified by designated characters and/or colors. The asterisk (*) character in the result tells the clinician that a comment exists on the result. The corrected, (c), characters are also added for results that have been corrected or updated by a user within the system. RESULTS REVIEW TAB

The Summary Documents Tab functions in the same manner as the All Documents Tab. However, on the Summary Documents Tab, you are only able to view pertinent clinical documents (such as operative reports or H&Ps). (In contrast, the All Documents tab displays all documents entered into the system.) SUMMARY DOCUMENTS TAB

The All Documents tab displays all clinical documents that have been entered for the selected patient. Information such as Emergency Room reports, visit notes, procedure information, and H&Ps can be viewed from the All Documents tab. Toolbar The view of the All Documents Tab is broken up into: the Index Tree, Toolbar, Navigator Bar, and Viewing Window. Adjusting the Date on the Navigator Bar You can filter the display of documents by specific encounter, date range, or document count. To filter the display of documents, right click on the navigator bar and select Change Search Criteria. In the Clinical Notes lookup window, specify the correct criteria. Then press OK. Documents display based on the date range, encounter, or document count. If your filters are not set properly, some documents may not display. ALL DOCUMENTS TAB

Lesson Four: Viewing a Patient’s Chart Use the Visit List sub-tab to view a patient visit summary to CHKDHS. The Visit List is available from the patient chart as a tab of the Patient Information view. Select another visit by double-clicking the selected encounter or clicking on Options on the menu command toolbar and selecting Change Encounter. PATIENT INFORMATION TAB

Lesson Four: Viewing a Patient’s Chart Use the Patient Demographics sub-tab in the Patient Information view the same way that you would a paper chart’s face sheet. You can view information such as name, address, phone number, gender and any previous names the patient may have. Demographic information recorded in Eclipsys during the registration process is displayed here. Blank fields are an indication that those fields may not have been populated during the registration process, or that specific information is not recorded in PowerChart. PATIENT INFORMATION TAB

Lesson Four: Viewing a Patient’s Chart The Patient Provider Relationship Summary (PPR) Sub-tab in the Patient Information view is used to display a patient's relationships with healthcare providers known to CHKDHS. Separate lists are provided for Lifetime and Visit Specific Realtionships. This tracks all users that have accessed the patient’s record. Do not access patient records unless you have an appropriate relationship to do so! This information is displayed in a split screen. The top portion displays established lifetime relationship information. The bottom portion of the screen displays visit-specific relationship information PATIENT INFORMATION TAB

The MAR Summary displays a condensed view of medication administration information to give the user a high level overview of the medications that are prescribed for the patient. It is mainly used by physicians and by nursing staff during change of shift report. MAR SUMMARY TAB

Lesson Four: Viewing a Patient’s Chart The Immunization Schedule Tab has multiple panes to assist you in viewing current and past immunizations. The view is divided into two components: the Previous Immunizations window, and the Future Immunization Schedule window. IMMUNIZATION SCHEDULE TAB

32 ADVANCED GROWTH CHART TAB
Lesson Four: Viewing a Patient’s Chart From the Advanced Growth Chart Tab, you can view height, weight, head circumference and BMI information for your patient. In addition, you can use zoom functionality and calculations of exact percentile on data points. The Advanced Growth Chart Component will open automatically in a split-view. Growth charts for females display in red, while growth charts for males will display in blue. ADVANCED GROWTH CHART TAB

The Overview Tab provides a quick summary of charted data for the selected patient. It is designed to present a summarized view of patient data that has been recorded since the last time the Date/Time Stamp button was clicked. If needed, you can quickly access detailed information displayed in the overview by clicking on that item. Overview Tab

When the Date/Time Stamp is selected, all results will be cleared until new results that have not been viewed by the user are present. Overview Tab

The Appointments Tab will display a list of the selected patient’s future and past appointments from Epic interfaced schedules and SurgiNet. The patient’s appointment information is displayed in a split screen. Future appointments are displayed at the top and past appointments display at the bottom of the screen. Past appointment information is purged from the Appointments tab after 365 days. Appointments

The QuickNotes tab will be available only on patient’s charts that are live with the EMR. Information such as nicknames, contact information, device information and clinical reminders can be entered on this chart tab for quick access. This Milestones section of QuickNotes is for general notes for the patient or for noting important milestones that occur during the patient’s hospital stay (i.e. date of extubation, first solid food intake), etc. QuickNotes Tab
